0158665082 2016-01-25 Drill / Deepen / Reopen No Violations On this day there was not activity on the site. This well is drilled. There is no production equipment currently on this site. Site appeared to be in satisfactory condition.
0637890598 2017-08-24 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to sand separator and a gas processing unit and a and then to a production area and to sales. The production area contains four 400-barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
0946444863 2015-08-17 Casing String Witnessed No Violations On this day Lightning Energy had drilled the 36 open hole to a depth of 100. The 30 casing was ran and cemented. Allied Cementing was on site and pumped 275 sacks of Class A cement with cement to surface established. See cement ticket for details.
1482464969 2015-08-28 Casing String Witnessed No Violations My inspection today finds the rig has been unable to land the 13.375 casing string. The casing set down five joints early. After washing one joint in the rig has been unable to wash the casing in deeper. Paul Hahn the XTO drilling superintendent decided to pull the casing out of the well and make a clean out run with the drill pipe.
1486182339 2015-08-21 Casing String Witnessed No Violations On this day First Class Energy had reached a total depth of 525. The 18.625 casing was ran and cemented to a depth of 503 and C&J was on site to cement the casing with 1105 sacks of Class A cement. Cement to surface was established following the top out job. A pressure test was conducted and passed the 300 # for 30 minutes statute. See cement ticket for details.
1544023250 2018-12-04 Production Wells No Violations On 12/04/2018 I visited the site with Inspector Ryan Armstrong. The inspection purpose was to conduct an Urban Pre Site inspection due to the company proposing (2) additional wells on the pad. Upon inspection (2) producing wells were found. The well pad was constructed as a post construction rule pad and one material modification was found on file addressing an earthen slip on the outward South East facing slope. The well pad is not equipped with a perimeter boundary earthen berm. There is a gate across the lease road access point. The urban waivers pertaining to the (2) additional wells on the pad have been waived.

2119110656 2018-12-04 Production Wells No Violations On the day of my inspection I witnessed that the well was currently using flow back equipment including a sand trap and choke manifold system, the sand trap was connected to 1 500-barrel mobile fluid storage tanks. The well head was equipped with 5.5-inch diameter production casing, and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place but had no digital readouts available for reference, there were annular pressure bleed down lines on both the 9.625-inch diameter casing, and 13.375-inch diameter casing, at the time of my inspection the 9.625-inch diameter casing valve was closed, and the 13.375-inch diameter casing valve was open and flowing through a choke manifold. Production equipment included a gas processing unit and 4 400-barrel production fluid tanks that were shared with the other wells on the pad. There was secondary containment around the production fluid tanks and gas processing units. There was a midstream valve site on the north side of the pad west of the lease road. Identification tags were present, and production reports were current.
