0733592749 2015-07-08 Plug / Plug Back No Violations This well is a lost hole. Following the 18.625 cement job the operator was tripping back into the hole with the 17.5 hammer bit when they encountered an obstruction @ 135. Upon further investigation it was found that the 18.625 casing had parted. The operator decided to plug this well. Cement was tagged when running tubing @ 155. The cement from the backside had entered the inside of the casing. Cement circulated top of cement to surface.
0866486712 2015-06-29 Drill / Deepen / Reopen No Violations Following the 18.625 casing job Alpha Hunter # 9 attempted to trip into the hole with their 17.5 bit. They hit an obstruction @ 130. Upon further investigation it was found that the 18.625 casing was compromised at that depth. The operator has decided that it is unfeasible to salvage this well and they have chosen to plug and abandon it. They will survey another well within 50 of the original location and permit it as the 3HA well.
1387171067 2018-01-02 Final Restoration No Violations The final restoration for this well is complete as the well has been plugged. The restoration for the well site pad disturbance area is required to be remediated once all other wells on this pad are plugged. Final restoration passed.

The 18.625 casing was ran and cemented into a 24 open hole to a depth of 340. O-Tex Cementing was on site to cement with 680 sacks of Class A cement. Cement to surface established.
