0818160218 2017-08-10 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to four 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
0995945311 2018-05-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place for the 9.625 inch diameter casing gauge read 100 pounds per square inch, the 13.375 inch diameter casing gauge read 25 pounds per square inch. Production equipment included a separator, Gas Processing unit, line heater, and 4- 400 barrel purge tanks shared with 3401320919, 3401320917. Identification tags were present and production was up to date.
1428677212 2015-04-08 Drill / Deepen / Reopen No Violations On this day Stingray Cementing was on location to attempt a pressure test on the 20 surface casing. The test was not successful. A slick line was ran into the hole to determine if the well had a competent shoe. There was 5-6 of cement inside the casing verifying no problems with the shoe. Company plans to run a bond log.
1738444243 2015-04-09 Drill / Deepen / Reopen No Violations On this day Warrior was on site to run a bond log to determine if the cement job provided a successful bond between formation and casing. The log was evaluated by Warrior, me, and an on-site Gulfport representative. The log showed competent bond between surface casing the formation.
2021177497 2019-01-10 Production Wells No Violations Status check inspection found (3) wells present on the well pad, all wells are constructed with A&B Sections and production trees and tubing head caps present ton the B section present indicating tubing is set in the well bores. 3 manual and one hydraulic valve is present on the production tree consist. Riser pipes equipped with manual gauges are present on the A&B sections for the purpose of monitoring annular pressure. All annular pressure on each well head was checked with no pressure noted on each gauge. The flow line pressure was 220 pounds on each well. Emergency shut down devices are present at each well along with at the production facility and the gas processing units and at the entry point of the location. The production fluid facility consists of (4) steel 400 barrel capacity tanks along with a 50 barrel tank, all situated inside a polymer lined steel containment dike area. There is hazard warning labels on the production facility. 3 gas processing units are present along with a mid stream pipeline take point at the right front corner of the well pad. The lease road is gated near the public road. The lease road and well pad surface is stone surface construction and in good repair. The well pad perimeter is protected by an earthen berm. The preliminary restoration is due and the well pad along with the lease road appears to meet ORC 1509.072 however will have to be re inspected when there is not snow on the ground.
2147273027 2015-05-11 Drill / Deepen / Reopen No Violations The pressure test on this well has failed. The company is required to run a bond log to determine if any remedial cement work can be completed. Remedial actions will be determine following the bond log interpretation.
