0373432480 2018-09-11 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in normal production operations. The well was equipped with 5.5-inch diameter production casing, and 2.375-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place but had no digital readouts available for reference, there were annular pressure bleed down lines on both the 9.625-inch diameter casing, and 13.375-inch diameter casing, at the time of my inspection the 9.625-inch diameter casing valve was closed, and the 13.375-inch diameter casing valve was open and flowing through a choke manifold. Production equipment included a gas processing unit and 2 400-barrel production fluid tanks that were shared with the other wells on the pad. There was secondary containment around the production fluid tanks and gas processing units. Identification tags were present, and production reports were current.
0530997815 2017-08-28 Production Wells No Violations This well is equipped with 5.5-inch production casing and appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area. The production area contains two 400-barrel production fluid holding tanks. The production is then routed to sale from there. 1185378846 2016-01-27 Completion Testing No Violations I received a call from Stan Lower of XTO. He informed me that XTO plans to begin flow-back operations on this pad.
1551201021 2019-02-26 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control and vegetation established. The well pad and lease road surface are stone construction and in good repair. No permiter berm is present around the well pad. One single well is present on the pad equipped with annular pressure monitoring transducers on the well head A&B Sections. The production facility consists of (2) steel 400 barrel production tanks situated inside a polymer lined steel containment dike area. A single gas processing unit is present.
1787621600 2015-09-19 Casing String Witnessed No Violations On this day First Class Energy Rig # 1 had reached the total depth of this section @ 430. The 18.625 casing was ran and cemented into a 26 open hole to a depth of 425. C&J Energy Services cemented the casing with 1200 sacks of Class A cement. A top out job was completed by running a 1 grout string to the top of cement found @ 185. Cement to surface was established. See cement ticket for details.
