0557612650 2018-03-29 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 450 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 200 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

-off from the well bore. Stingray Cementing preformed the cement procedures pumping 1450 sacks of Class H cement with returns to surface established.

0934670835 2016-01-21 Surface Facility Construction No Violations On this day I was accompanied by Emergency Responder Josh S. Josh and I were reviewing the slip area that is to be remediated. There was a track hoe working large rocks into the slip area for stabilization. Operations on this pad are on hold until the slip area is approved by the engineering section.
1027861625 2017-10-12 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains four 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
1134103858 2016-01-19 Surface Facility Construction No Violations On this day I was accompanied by Inspector Jim Nicholson. Jim and I were reviewing the slip area that is to be remediated. There was a track hoe working large rocks into the slip area for stabilization. Operations on this pad are on hold until the slip area is approved by the engineering section.
1158521813 2016-03-18 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. H&P Drilling 628 ran 357 of 18.625 casing. Stingray cemented the casing in place with 540 sacks of class H cement. I witnessed 17 barrels of cement circulate to surface. I witnessed the casing integrity test.
1375692312 2016-04-07 Casing String Witnessed No Violations On this day the operator was running and cementing the 5.5 production casing. Followin the completion of the cementing proceedures the rig will skid to the 5 B well and begin well construction.
1457353243 2016-02-29 Casing String Witnessed No Violations On this day Rocky Mountain had reached a total depth of 110 with the 32 auger bit. The 26 casing was ran and cemented by Stingray Cementing using 290 sack of Class A cement. Cement returns to surface were successful.

1610420931 2016-02-29 Casing String Witnessed No Violations On this day Rocky Mountain had reached the total depth of the 32 open hole auger section. The 26 conductor pipe was ran and cemented @ a depth of 110. Stingray Cementing Company was on site to cement the casing with 290 sacks of Class A cement. Cement returns to surface were established.
1630579513 2019-12-23 0:00 PW No Violations 1.00
2019951425 2018-07-31 Preliminary Restoration No Violations Preliminary restoration status check inspection found the area affected by the well pad construction has erosion control in place and vegetation established, the well pad surface and lease road are of stone construction and are in good repair. The entry point of the well pad is gated and locked with proper well site identification posted at the entry point of the lease road. There is an earthen berm established around the well pad and the company is controlling any collected surface water from rain events. There are a total of 6 wells on the pad that are in production at this time. The company has emergency shut down master control buttons and fire extinguishers place at all critical points on the well pad. The well head assembly consists of a lower B section and upper A section along with a production tree assembly. There are riser pipes with valves and manual pressure gauges attached to the A & B section that is allowing the company to monitor annular pressure. The pressure on the A section which monitors the pressure between the 13.375 diameter surface casing and 9.625 diameter intermediate casing string is currently 380 pounds per square inch (psi) and the pressure on the B section which is the pressure between the 5.5 diameter production casing and the 9.625 diameter intermediate casing is 740 pounds pressure. The 5.5 diameter production casing internal working pressure is 1000 psi and the production flowline pressure is 750 psi. There is an emergency shut down master control device on the production flowline downstream from the production tree manual control valves. There is a total of 6 gas processing units on the well pad one for each well. On the back right hand corner of the well pad is the production fluid storage tank farm consisting of (4) steel construction 400 barrel tanks situated inside a polymer lined steel containment dike area.
2086050778 2018-08-20 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with 5.5-inch diameter production casing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 150 pounds per square inch, and the 13.375-inch diameter casing gauge read 140 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and four 400-barrel production fluid tanks, and a 50-barrel produced water tank shared with the other producing wells on the well pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
