0457947328 2018-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 20-inch diameter surface casing, 13.375-inch diameter intermediate casing, 9.625-inch diameter deep intermediate casing and 5.5-inch diameter production casing, the well was currently in flow back operations with 2 sand traps and 2 choke manifolds. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 655 pounds per square inch, the 13.375-inch diameter surface casing gauge read 2.75 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel purge tanks shared with 34013210760000, 34013210750000, 34013210740000, 34013210720000, 34013207460000, 34013207470000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
0791028217 2018-07-10 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. The well site entrance point was marked with signage and the lease road was of gravel construction. There is 2 well pads sitting next to one another back the same lease road with the Gold Digger being the rear pad and the Sonuvadigger being the pad closest to State Route 9. There was recently 5 wells newly drilled on the pad and 2 prior producing wells. The said well was equipped with electronic annular pressure monitoring on both the 13.375 and 9.625 well head sections. The well was selling off the 5.5 production casing with 2 manually controlled 7.187 diameter master valves in place on the well head followed by an emergency shut down valve with multiple shut down point on the well pad. All valves were open and the well was flowing through a choke manifold and sand trap separator with 2 choke manifold and sand trap systems present on each of the 5 recently completed wells that are on flow back. Also present on location is one gas processing unit present for each well supported by a production fluid facility consisting of 5 steel tanks of 400 barrel construction design situated inside a polymer lined steel containment dike area.
0829608446 2016-09-18 Casing String Witnessed No Violations The purpose of my inspection today is to witness 100 feet of the 30 inch diameter conductor being cemented into place in a 36 inch diameter bore hole. Throughout the drilling process the bore hole did not run through any fresh water. During my inspection Butchs Rat Hole, O-Tex Cementing, and Rice Energy representative Jimmy Groves were on location. O-Tex began with a pressure test. Following the pressure test O-Tex pumped 5 barrels of gel, 5 barrels of water, and cemented with 51 barrels of Class A cement at 15.6 pounds per gallon. O-Tex pumped 71 barrels of displacement and cement came to surface. An estimated 4 barrels of cement came to surface.
13800870 2020-3-3 0:00:0 PW No Violations 0.20
1477065854 2016-10-19 Drill / Deepen / Reopen No Violations On location to witness the cementing of the 20 surface casing. Nomac rig 82 Ran 20 95# R-3 BTC casing to 508. O-Tex pumping cemented the casing with 790 sacks of Class H cement. See the casing ticket for the details.
1727490131 2017-09-12 Completion Testing No Violations I received a call from Terry Whitehair concerning a spill that had taken place on this location. The company is currently in the completion phase. Stingray was on site and providing the pumping service for completions. During operations, there was a mechanical failure that resulted in the alternator malfunctioning which caused the blending tank to over fill. There was an estimated 2.5 barrels of production water that was released onto the pad. Water trucks on site had vacuumed the fluid up and completion operations were shut down. The company will resume operations and they planned to be completed within four days. Following the mobilization of equipment off-site, the company will evaluate the impacted area and provide further remediation where necessary.
1915718947 2017-09-06 Production Wells No Violations Completion operations are currently being conducted on this well. The company is stimulating the well bore by utilizing hydraulic fracturing.
791028217 2018-7-10 0:00: PW No Violations 0.10
