0042455813 2017-10-12 Casing String Witnessed No Violations physical address: 62827 johnson south road, bethesda oh 43719. On the day of my inspection I witnessed Express energy had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express was onsite with 275 sacks of class A cement. Additives include 2% CaCl, and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50% percent excess ran. Cement was displaced with 70 barrels of fresh water. 12 barrels of cement returned to surface.
0763666030 2018-06-14 Casing String Witnessed No Violations On the day of my inspection I witnessed that Patterson UTI 582 had drilled the 24 inch diameter hole to a depth of 385 feet. Franks Casing crew ran 377 feet of 18.625 inch diameter J 55 casing at 87.5 pound per foot, with 3.84 feet of casing above Kelly Bushing, which is 25 feet above ground level. BJ Services was on site and rigged up their cement head, loaded lines with 5 barrels of fresh water and pressure tested to 2500 pounds per square inch. BJ Services established circulation with 225 barrels of fresh water, pumped a 35 barrel gel spacer, and 10 barrels of fresh water spacer. BJ Services then pumped 155 barrels 727 sacks of 15.6 pounds per gallon with a yield of 1.20 cubic feet per sack Class A cement mixed with 5.24 gallons of water per sack. Cement was displaced with 101 barrels of fresh water and had 25 barrels of good cement returns to surface. Plug was bumped at 100 pounds per square inch and pressured up to 600 pounds per square inch and held for 5 minutes, floats held with .5 barrel of flow back.
1108230801 2018-06-28 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P 471 was currently drilling ahead at 8625 feet on the curve section of the 8.5 inch diameter well bore, with an estimated Total Depth of 25,900 feet. Pat Smith the company man for EQT informed me that the 9.625 inch diameter casing was set on 6/25/2018, and cemented on 6/26/2018 with good cement returns to surface. I performed a Rig Check and found no violations at the time of my inspection.

I spoke with company man Roger Leger and he informed me that they would begin drilling the surface section of the well bore at approximately 09:00 on 6/13/2018. Roger informed me that they would drill to an estimated total depth of 400 feet, and should reach total depth at around 24:00 on the morning of 6/14/2018. They plan to run 18.625 inch diameter casing to total depth and cement to surface. I performed a walk around and found no violations at the time of my inspection.

1345864260 2018-07-11 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik. I witnessed that H&P 471 had tripped in the hole to 8250 feet and increased mud weight from 13.2 pound per gallon to 14.5 pound per gallon oil-based mud. After speaking with company man Roger Leger, he informed me that the well was currently shut in with annular preventer and they were holding 550 pounds per square inch of confining pressure on the well bore. H&P 471 was currently working on their fluid return system and adding a secondary pump to ensure good circulation to the mud containment pits. Roger informed me that once fluid system is corrected they will test for flow and confirm the well is static. If well is static they plan to trip out of hole and change current bottom hole assembly to a clean out bottom hole assembly and trip back in the hole circulating 14.5 pound per gallon mud weight approximately every 5000 feet through the entire well bore.
1492204470 2019-06-04 Completion Testing No Violations On the day of my inspection I witnessed Halliburton Pressure Pumping, Schlumberger wireline were currently on location and had just completed the 39th stage of the stimulation process out of a total of 84 total stages. At the time of my inspection the stimulation process was currently shut down due to a wireline failure at surface. Annular pressures were being monitored on every well on the pad. After speaking with EQT consultant Brad Wolff he informed me that they had a Schlumberger wireline failure at surface, they intend to switch out the Schlumberger wireline truck and tool. I informed EQT consultant Brad Wolff to keep me updated with any problems or situations that occur during the stimulation process.
1531250948 2018-07-10 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found H&P drilling rig 471 on location and rigged up on the well. The rig has reached a amended total drilling depth of 25,984 in the 8.5 production borehole. The rig was recovering the drill string at a total measured depth of 2000 when they started to gain fluid in the drilling pits, a total of 2 barrels of fluid was gained and the blow out preventer was shut in. The company is going to run the drill string back to the 9.625 casing shoe at 8,245 and increase their mud weight from 13.2 pound per gallon to 14.2 pound per gallon. Once they have the well bore at a static level they will pull the drill string and remover the directional drilling assembly then run back in the hole to total depth and condition the well bore with the increased fluid weight before pulling the drill string once again in preparation of running the 5.5 diameter production casing.
1531314223 2018-07-10 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that H&P 471 was currently tripping in the hole at 3200 feet through the annular preventer. Pat Smith Company man for EQT called and informed me that at approximately 03:30 on 7/10/2018 the draw works motor failed and while waiting on a new draw works motor they took a 2-barrel gain. They were currently at 13.2 pound per gallon oil-based mud in the well bore. H&P 471 then shut the well in to monitor pressure increase, original shut in pressure was 80 pounds per square inch and after 1-hour pressure had increased to 120 pounds per square inch, after conducting a flow check they determined that they were getting approximately .4 barrels per hour flow. Company man Roger Leger informed me that they plan to trip in the hole to 4000 feet and monitor pressure for 30 min to determine if pressure is still increasing. If pressure is increasing they plan to trip to 8250 feet just below the 9.625-inch diameter casing shoe and increase mud weight to 14.2 pounds per gallon. If well remains static they will trip out of hole and lay down current bottom hole assembly, and trip back in hole to total depth with a clean out bottom hole assembly and circulate entire well bore with 14.2 pound per gallon oil-based mud.
1531315806 2018-07-11 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process with H&P Drilling rig 471 on location. On this day I was accompanied by ODNR Inspector Ryan Armstrong. The rig has reached a total drilling depth of 25,941 in the 8.5 diameter production borehole. While pulling the drill string at a depth of approximately 2.000 they experienced a 2 barrel gain in the mud pits. The BOP was shut in and the circulating fluid weight increased from 13.2 pound per gallon to 14.5 pound per gallon and they are holding a 550 pound per square inch applied pressure on the well. The company is adding an additional circulation pump on their circulating return side and once the pump is in line they will circulate the well bore at the current drill string depth of 8,250 which is 5 below the 9.625 casing shoe.
1551728823 2019-03-04 Completion Testing No Violations Status check inspection found 6 wells present on the pad. The said well has been drilled and not completed. The well is equipped with a night cap with a manual gate valve on the night cap. There is a gauge on the valve but the valve is closed. No annular pressure monitoring is present on the A or B section. The Del Scorcho # 8 #10 and #12 wells have been drilled but are not completed. The number 2 #4 and # 6 wells are in the flow back production stages and are not equipped with annular pressure monitoring either. There is a large above ground storage tank on location. The well pad perimeter does have a sound wall erected on top of the earthen berm. A third party midstream pipe line was installed ant the earthen berm was removed and not properly put back. I did notify ODNR Engineer Technician Anthony Carson on this day of the deficiency related to the berm.
