0436179634 | 2018-08-23 | UrbanProduction Wells | No Violations | Status check inspection found the well pad perimeter boundary protected by an 8 chain link fence that is gated and locked. There is also a gate on the lease road with signage posted at the well pad entrance points and at each well head with hazard warning labels on the tank battery. The lease road and well pad surface are stoned and in good repair. The well head assembly consists of a lower A section and upper B section with 2 diameter riser pipes on each section access ports with valves and gauges present to monitor annular pressure. The A section gauge which monitors the pressure between the 13.375 and 9.625 diameter casing strings reads zero pounds pressure and the B section gauge which monitors the pressure between the 5.5 and 9.625 casing strings reads 1,300 pounds pressure. The 5.5 diameter casing internal pressure is 1,900 pounds and the production flow line downstream of the well head production tree and choke reads 1,000 pounds pressure. The wall head production tree has multiple manual shut down and one emergency shut down valve with emergency shut down control points at all critical areas on the well pad including the entry point. There are a total of 4 wells producing on the pad and 4 gas processing units. The production fluid storage area consists of (4) steel construction 400 barrel fluid capacity tanks and one single smaller tank approximately 70 fluid barrels volume, all situated inside a polymer lined steel containment dike area. Also present in the production dike is a charcoal filter. At the entry point of the pad is a midstream facility operated by EQT. |

1494087537 | 2017-05-06 | Casing String Witnessed | No Violations | On site to witness running and cementing of surface casing,Nomac Drilling drilled to 379 of 24 hole and ran 383.75 ft. of 18.625 casing and Stingray cemented it in place with 600 sacks of cement and they had 28 barrel of good cement returns.
Verified cement mix water to be fresh waterand inspected casing prior to running it in hole checking for any potential defects. |
1495226387 | 2017-05-16 | Casing String Witnessed | No Violations | On site to witness running and cementing of 9.625 casing Nomac rig 137 drilled to a depth of 8961 of 12.250 hole and ran 8946 of casing and Stringray cemented it in place with 2724 sacks of cement string was equipped with 1-float collar and 1-guide shoe and 22 centralizers. |

1950723283 | 2018-07-30 | Production Wells | No Violations | On the day of my inspection I witnessed that the well was equipped with a three-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was open flowing. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 1290 pounds per square inch, and the 13.375-inch diameter casing gauge read 0 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel production fluid tanks, and a 50 barrel produced water tank shared with 34013211010000, 34013212290000, 34013212310000. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date. |
2125462937 | 2018-03-13 | UrbanProduction Wells | No Violations | The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 200 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1500 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . There was a CS Trucking tank on site pulling fluid from the production tanks. The production records within RBDMS are currently updated to 2017. |
