1502687672 2017-08-14 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Patterson Drilling rig 583 on location along with Stingray Cement Service. The rig has drilled a 17.5 diameter surface borehole to a measured depth of 1,678 and ran 1,678 of J-55 Buttress thread casing with a guide shoe and float collar casing equipment ran. Stingray Cement Service cemented the casing using a 307 barrel cement mix consisting of 1,500 sacks of class A cement along with 0.3% Calcium Chloride and 0.25 pound per sack flow seal. The cement weight was verified by weight of scale at 15.6 pound per gallon. 25 barrels of 6% gel spacer was ran ahead of the cement and the drilling rig loaded the hole with fresh water upon reaching total depth before pulling the drill string. The rig also circulated the last casing joint into the well bore. The cement was displaced to the float collar with 253 barrels of fresh water and the casing was pressure tested to 1,170 psi. The final displacement pressure was 650 psi and 43 barrels of cement returns were witnessed to surface.
1716950109 2019-02-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the lease road and well pad surface is stone construction, the well pad is in good repair but the lease road is starting to deteriorate due to the freezing and thawing conditions and heavy traffic. Signage is posted at the entrance of the lease road a on each of the (2) wells present on the pad. The well pad per miter has been recently disturbed by the installation of the 3rd party gas sales lines and the areas disturbed will need to establish vegetation before the restoration can be considered complete. The said well is equipped with a lowered A and upper B section, attached to the well head sections are 2 diameter riser pipes. The pipes are equipped with valves and gauges along with electronic transducers for the purpose of monitoring the well bore annular pressure. Crews are on site installing the final production equipment on the well head, the well drilling completion and the initial production flow back stages are complete and the well is being turned over to the production section of the company. The company representative on site indicated the wells were shut in during the final hook up stages and the wells were equipped with 2.875 production tubing internally. A the left rear corner of the well pad in the production facility that consists of (4) steel construction 400 barrel capacity tanks, all which are situated inside a polymer lined steel containment dike area. Fire extinguishers and emergency shut down devices are situated at all critical areas of the well pad.
