0072571782 2019-06-12 Preliminary Restoration No Violations Status check inspection found a total of 5 wells present on the well pad. 3 were previously drilled and produced and 2 were recently drilled. The 2 recent wells have been drilled and are awaiting stimulation. Both wells were constructed and set inside a Cellar Tech cellar below ground level elevation. The well pad restoration meets ORC 1509.072 B.
0818598537 2019-05-14 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson UTI 292 had drilled the 12.25 inch diameter well bore to a depth of 8,675 feet and had ran 8,660 feet of 40 pound per foot J55 9.625 inch diameter buttress thread casing with 3.9 feet of stick up with the Kelly bushing at 28.5 feet from ground level, a DV tool and external packer were used with the DV tool set at 5,408 feet. At the time of inspection Schlumberger was on location and had already pumped the first stage of the cement job. Schlumberger was currently pumping 2 bottoms up to clean the well bore above the DV tool. Schlumberger then pumped 427 barrels of 15.6 pound per gallon class a cement with a yield of 1.26 cubic feet per sack with 5.25 gallon of mix water per sack. Schlumberger shut down dropped a plug and displaced with 410 barrels of 13.5 pound per gallon oil base mud, with 90 barrels of cement to surface. Plug was bumped at 1000 pounds per square inch and pressured up to 2500 pounds per square inch. Floats held with 4 barrels of flow back. I informed EQT consultant Matt Kiter that casing must remain static until cement reaches a compressive strength of 500 pounds per square inch.

1552670515 2019-03-15 Casing String Witnessed No Violations On site inspection for a 8:00 am call out to witness the conductor casing cement job with a 3rd party top hole rig on location owned by Cellar Tech, also present id a cement service identified as Lightning Energy. The company has set a Cellar Tech Cellar the measured 9 deep and 7 x 10 square. The rig drilled a 40 diameter conductor borehole to a measured depth of 100 and set 100 of 30 .375 wall conductor. The conductor was cemented using 96 barrel 457 sack Class A cement mix with the cement weight verified at 15.6 pound per gallon by weight of scale.30% cement excess was ran. The cement was displaced with 73.5 barrels of fresh water. Cement was witnessed back to surface inside the cellar, the cement stayed up to surface. The company man called me out for the job at 8:00 am but it did not start until 14:00 pm.
1556740038 2019-05-01 Casing String Witnessed No Violations On the day of my inspection I witnessed that Patterson UTI rig 292 had drilled the 24-inch diameter hole to a depth of 650 feet. Wyoming Casing crew ran 604.7 feet of 18.625-inch diameter J 55 casing at 87.5 pound per foot, with 4.7 feet of casing above Kelly Bushing, which is 28 feet above ground level. Cement basket was set at 430 feet. Boss Services was on site and rigged up their cement head and loaded lines with 5 barrels of fresh water and pressure tested to 1500 pounds per square inch. Boss Services established circulation with 190 barrels of fresh water and a 50-barrel gel spacer, washed up and then pumped 233 barrels 1155 sacks of 16.0 pounds per gallon with a yield of 1.13 cubic feet per sack Class A cement mixed with 4.74 gallons of water per sack. Cement was displaced with 170 barrels of fresh water with 30 barrels of good cement to surface. Plug bumped at 275 pounds per square inch and appeared to have failed, an additional 5 barrels of fresh water was pumped, and pressure did not increase valves were opened and it appeared the floats held with 1.5 barrels of flow back. I informed EQT consultant Matt Kiter that the casing must remain static until cement reaches a compressive strength of 500 pounds per square inch at which time they will have to perform a casing integrity test of 300 pounds per square inch hold it for 30 minutes.
