There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complet

0287331318 2018-09-17 Production Wells No Violations No equipment changes have been made to the pad since my last inspection. The surface-intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 361 psi. I performed a walk-around inspection of the pad, and no violations were observed at this time.
0393471816 2018-04-23 Production Wells No Violations I conducted a status check of the pad. The pad is equipped with eight producing wells, and partially drilled wells with conductor casing (# 22624 & 22623). The surf/int pressure gauge displayed a reading 0 psi, and the int/prod displayed 50 psi. The tank battery consisted of six -400 bbl production tanks. Other equipment on the pad consisted of the following.




The access road was stable. No violations were observed at the time of inspection.

0920791426 2014-12-01 Drill / Deepen / Reopen No Violations Nomac Rig 77 drilled a 17.5 hole to a depth of 957 and ran 945.74 of 13.625 54.5 lb./ft. of J-55 surface casing. O- Tex Cementing pumped 30 barrels of fresh water gel ahead and started mixing there cement in the tub to get the cement weight up to 15.6 lb./gallon. While they were mixing the cement lines from the cement bulk trucks to the pump truck plugged off. After taking the lines apart they noticed that it was plugged with cell flake. After talking with the O-Tex and Rick the Cement Consultant for Chesapeake and Terry Lambert the Company Representative the decision was made to sent the bulk cement back to O-Tex yard and blend a new batch of cement (890) sacks. No cement was pumped down the casing. After a few hours O-Tex returned with a new batch of blended cement. O-Tex pumped 890 sack of Premium H Cement with 2% CaCl2, 0.4C-41P .25 lb./gallon of Cello-flake. Cement was weight by pressurized cement scales at 15.6 this was witnessed by myself and Steve. O-Tex pumped 131.4 barrels of fresh water to displace the plug floats held. There was 15 barrels of 15.8 lb./gallon of cement returned to surface.
1390524672 2018-11-08 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 47 psi. I performed a walk-around inspection of the production pad and no violations were observed at the time of inspection.

1419433877 2014-12-02 Completion Testing No Violations Nomac Rig 77 drilled a 12.25 hole to a depth of 1797.35 and ran1794 of 9.625 40 lb./ft. J-55 intermediate casing. O-Tex pumped 35 barrels gel spacer and 6 barrels of water spacer, followed by 630 sacks on Premium H 3% Gypsum, 2%CaCl2, 0.8% MC-500 (BWOW), 0.5% C-45, 0.4% C-41P. O-Tex pumped 129 barrel of water to displace the plug. Plug landed and floats held. There was 20 barrels of 15 lb./ gallon of cement returned to surface.
1682170086 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.
