1429649383 2015-03-14 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #71 drilled a 17.5 hole to a total depth of 671 and Wyoming Casing set 663 of intermediate casing in the hole. Schlumberger rigged up, tested the lines, and cemented the casing with 131 barrels of Class A cement slurry. They released the plug and displaced the plug to the float collar with 96.8 barrels of freshwater. The plug landed at 430psi and the pressure was increased and held at 930psi for 5 minutes. The pressure was released and 0.5 barrels flowed back to the truck. 35 barrels of cement circulated to surface. No violations. See cement ticket for more details on this job.
1456759916 2016-02-22 Production Wells No Violations I conducted a status check inspection of this Utica Shale well pad. My inspection found the lease road gate present and open due to ongoing production operations. The lease road and apron are in good repair. The pad is fenced and the entrance gate was open and all man doors on the perimeter of the pad were locked. A Pilot tanker truck was picking up a load of oil/condensate at the tank battery. The #5H well is in production and the gauge on the tree cap indicates a pressure of 343psi. The 5.5production casing wing valve indicates a pressure of 748psi. The well is choked back and producing at a pressure 313psi to the surface production equipment. Well #s 6H (PN# 22652), 7H (PN# 22653), 8H (PN# 22651), and 9H (PN# 22629) are also in production and producing. The proper IDs are posted at the entrance and at the wellheads and tank battery. The pad is equipped with video surveillance.

The equipment on the pad consists of the following:

5 Line heaters (disconnected and not in use) 2 Heated oil stabilizer units 1 Thermo-electric desiccator with a vertical drip bottle 1 Self-contained, 350 gallon, methanol tank containing Assure HI-18FB hydrate inhibitor 1 Metal compressor shed housing a natural gas powered compressor 1 Low pressure separator tower 1 Combustor with inline pressure tank and air compressor There is a line heater, a separator tower, and various production equipment parts on the northeast side of the well pad.

1726784473 2015-03-16 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #71. They are currently drilling the 8.75 vertical section at a depth of 4,590. This section will be drilled to a depth of 7,141 at which point they will mud-up and begin drilling the 8.5 curve and lateral. The location is in good condition and all drilling equipment is on containment. No violations.
2035170563 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.
