0118324828 2017-09-21 Production Wells No Violations
0218183625 2017-08-14 Production Wells No Violations I conducted a status check of this well while visiting the Goebeler B pad, where Yost #8 drilling was moving on site. The well is in production, and no changes were made to the pad since my last inspection.
0287814405 2017-03-15 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #1H well is in production and is equipped with an open flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of nine 400 bbl production tanks, three labeled as containing produced water, and six labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

9- 400 bbl production tanks, three labelled as containing fresh water, and six labelled as containing condensate 3 producing wells 3 sand separators 3 gas processing units 1 vertical separator 1 vapor recovery unit Barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The lease road was stable in in good repair.

- had been removed, and the well was shut-in with a 15k frac valve. The surface--production annular gauge displayed a reading of 967 psi. I contacted Eric Ross of Rex Energy to discuss the int--side pressure for review.

-intermediate pressure gauge displayed a reading of 930 psi. The annular pressure plots of the backside pressure received on 02/24/2018 showed fluctuations in the pressure, but did not appear to correlate to any stages on the B pad. I requested a daily back side pressure plot from the operator so that I may continue to monitor the pressure.

1195927410 2017-06-29 Production Wells No Violations I was on site with DOGRM Tom Hill and Terry Whitehair, along with R.E Gas Development personnel to conduct a pre-site for a proposed Goebeler B Pad. I performed a walk-around inspection while on site. Personnel were in the process of moving the location of the combustor unit in anticipation for the new B pad construction. I was unable to locate a clear surface-intermediate pressure gauge on the well. I spoke to R.E. Gas Development to have a gauge installed on the surface-intermediate annulus and labelled.
1404773691 2017-06-19 Production Wells No Violations I met on site with representatives of R.E Gas Development and DOGRM Engineers to discuss proposed construction plans to construct a pad adjacent to the Goebeler pad in order to drill seven proposed new wells. I conducted a status check of the pad while on location. The well is in production and is equipped with a plunger lift assembly. No violations were noted at the time of inspection.
1432456813 2016-03-10 Drill / Deepen / Reopen No Violations I was contacted at 0336 hrs. by Aaron Wilhite regarding a Synthetic Oil Base Mud ( SOBM ) spill that occurred while drilling at 10,531 MD. A rupture in the Kelly hose, while drilling with 3600 psi pump pressure, released approximately one (1) barrel of SOBM. The arreal extent of the spray hit the 5.5 production casing, V- door, center tub and rig floor. The SOBM spray was off the the equipment, casing and rig mats. A vacuum truck from KSW sucked up all the spilled SOBM. I arrived on location at 0830 hrs. and could not identify any of the SOBM contamination. The Kelly hose was replaced. I inspected the cellar, solids handling area, SOBM tanks, mud pumps and did not find any concerns No violations identified during this inspection.
1522285349 2017-10-11 Production Wells No Violations I performed a status check of the well. Sidewinder #62 was drilling the #11H well (PN#22742) at a depth of 9970. The surface-intermediate annular pressure gauge displayed a reading of zero psi, and the 9.625- 5.5 annular pressure gauge displayed a reading of 700 psi. The production equipment is surrounded by jersey barriers, and man trailers are located on the pad during drilling of the Goebeler B pad. No violations were noted at the time of inspection.

-lift unit. The surface--around inspection of the pad perimeter and erosional controls were in place and functioning. Production equipment consisted of the following: - 400 bbl production tanks, nine labeled as containing condensate, and nine as containing produced water

Three vapor recovery units Four oil stabilization units with towers and vertical separators Ten gas processing units with vertical separators Seven combustor units

1588397057 2017-10-17 Production Wells No Violations I conducted a status check of the well. Drilling operations were taking place on the Goebeler #8H well (34-019-22737-00-00). No changes have been made to the producing pad since my last inspection. The surface-intermediate annular gauge displayed a reading of zero psi. No violations were noted at the time of inspection.

1971009231 2018-02-14 Production Wells No Violations I was on site while completion operations were in progress for the seven wells on the B pad. The production T had been removed from the well head, and 15k frac valves were installed on the well which was shut-in. The surface-intermediate annular gauge displayed a reading of zero psi.
