0290986503 2014-10-15 Administrative Inspection No Violations Complaint ID# 4833. I received a call from Mr. Evans today (10/15/14) stating that the creek was hardly flowing the last several days and today it is flowing extremely high and is loaded with silt. I told him I would contact OEPA about the problem. I contacted Kurt Kollar with OEPA and e-mailed my findings along with some photos to him. He said he would pass this on to the Surface Water Division for inspection and compliance.
0331930590 2014-10-09 Non Well No Violations Follow-up to complaint #4833 concerning siltation in a creek. I accessed the creek 0.30 miles west of Mr. Evans property and east of the newly constructed right-of-way at the lease road for PN#22058. I did not notice any siltation in the creek east or west of this location. I did note high, unstable cut banks along the meandering creek. I inspected the creek at the culvert on SR 212 and found that the water was running clear. I will continue to monitor the situation.
1175110037 2015-04-28 Administrative Inspection No Violations I was on location with DOGRM Engineers Anthony Carson and Wayne Cottle, and Chesapeake Field Reps Steven Rybicki and Matt Garrison to conduct a pre-construction site inspection for this horizontal well pad. The location is in an agricultural field with drainage areas to the east and southwest. A pipeline tie-in was installed last year off the southwest corner of the staked pad. Topsoil will be stockpiled north of the lease road entrance and southeast of the well pad. Pad underdrains will be located on the north side of the pad and near the southeast corner. The Engineers will review the plans and complete the required paperwork. No violations.
1422981577 2014-10-08 Non Well No Violations Report in regards to complaint #4833. I received a call from Mr. Blaine Evans at the Uniontown Field Office at 1:07pm. He said he thought that nearby pipeline activity was polluting a creek that crosses his property. He said the creek water was always clear and now had the appearance of coffee with creamer in it. He also stated that he had spoken with other agencies and that no one has been out to investigate. I met with Mr. Evans at 3:00pm and investigated the creek at the culvert pipe running under State Route 212. The creek was flowing from west to east and was charged with a heavy silt load. I ran a low-level Quantab and found no elevated levels of chlorides in the water. I inspected a recently cleared right-of-way on Cordova Road SW 1 mile southwest of the culvert pipe. I found that Access Midstream has recently installed a new pipeline that crosses the creek at 2 locations. The activity is fresh and no stabilization of the soil has taken place. They have silt fence and hay bales in place, but appear to be overloaded on the northeast portion of the right-of-way near the creek. Recent, heavy precipitation may have entrained the silt from this location, but water in the creek was running clear at the time of my inspection. I will inspect other locations along the creek tomorrow. Right-of-Way GPS, NAD83, 40.44067, -81.22050: Culvert GPS, NAD83, 40.44067, -81.20363.
