access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #8H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 6 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 1 - Gas processing units (GPU) 1 - Vertical separator 1 - Sand separator

1 - Thermal electric generator 1 - Producing well Site fenced and man doors locked

0453066128 2017-08-21 Production Wells No Violations The purpose of my inspection was to conduct a status check on well#2-1773. The lease road and well pad remained in good repair. The topsoil stockpile remained stable. A locked chain link fence was constructed around the facility. No signs of vandalism were present. A stable earthen berm was present outside of the fenced perimeter. Correct identification was present. The well cellar contained fresh water near free board. No leaks were detected. The 9.625 annular pressure gauge indicated 2.5 psi. The 5.5 annular pressure gauge indicated 390 psi. The well was flowing during the inspection. The 5.5 production casing was equipped with a plunger lift assembly. Production equipment on the pad consisted of (1) sand knockout and GPU, one (1) hydrate inhibitor tank, one (1) meter run, one (1) thermal generator and three (3) 400 bbl produced water and three (3) 400 bbl condensate tanks inside steel containment. The containment was lined with an impervious liner and was holding fresh water. No violations were observed during this inspection.
0507093736 2017-01-30 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1773-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road was stable and drivable. No signs of erosion were observed adjacent to the road traveling to the pad. An earthen dike perimeter is erected around the well pad. A chain link fence with barbed wire top is constructed around the pad to prevent unauthorized entry. An Emergency Shut Down (ESD) button was adjacent to the main entrance gate. The main entry gate and escape doors positioned around the perimeter were found to be closed and locked. The soil stockpile was stabilized with well-
0557797908 2013-03-26 Surface Facility Construction No Violations I stopped at the site to check on the pad construction. Construction of the access road and pad were in progress. Considerable work remains for completion.
0589339259 2013-04-19 Drill / Deepen / Reopen No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 118. Express installed 118 of 20 OD casing. During drilling, water was encountered at an approximate depth of 30. Express Energy Services was the cementing contractor. The cement was mudscaled at 15.5 pounds per gallon (ppg), and adjusted to 15.6 ppg. Express pumped 51.49 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39.07 bbls of water. Cement returns to surface were observed. An estimated 10 bbls of cement returned to surface. Displacement was completed at 4:11pm.

-based mud used to drill and condition the hole, a lead cement slurry followed by a tail cement slurry were pumped. The lead cement was mudscaled at 13.5 pounds per gallon (ppg). Schlumberger pumped 335 bbls of lead cement at 13.5 ppg. Schlumberger then pumped 352 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 317.5 bbls of water as measured by tanks. The plug bumped the float collar at 5:10am on 05/13/2013. Pressure at plug bump was 2511 psi. Pressure was raised to 3120 psi, and held for 5 minutes. Pressure held. The float held when the pressure was released, and 3.5 bbls of water bled back. An estimated 11 bbls of the 60 bbl Mudpush spacer returned to surface.

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there is a site drain installed. Outlets are located at the south and west ends of the pad. Inside the fence, there is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and approved.

1582137025 2013-02-19 Surface Facility Construction No Violations I stopped at the site to check on the pad construction. Construction had not yet begun. Pipeline construction was in progress in the area.
1627754975 2013-05-01 Drill / Deepen / Reopen No Violations I went to the site and had a pre-spud meeting with drilling consultant Chuck Butler regarding the drilling of the borehole for the surface casing. The rig will provide a copy of the open hole log, both paper and electronic. The mud loggers will also provide their determination of the top and bottom of the Big Injun Sandstone. It is anticipated that the rig will begin drilling within an hour or so.
