0571516623 2015-10-13 Production Wells No Violations On the day of my inspection I found that the pumping unit had been equipped with a new motor and pulley belt. The polished rod was showed signs of recent use. I contacted James Miller and he claimed that they pulled the tubing, replaced the lower 16 joints with new 2.375 upset and installed a rebuilt central pump. The valves were closed and there was no pressure on the gauge. Miller claimed that there was 450 of fluid in the well, but they need to run a liner in the flow line before pumping because he didnt trust the integrity of the original line. He agreed to call me when they get the liner in so I can witness the well pumping.
1554486078 2019-04-05 Production Wells No Violations On the day of my inspection I found that the flow line had been repaired and there was no oil residue left in the surrounding soil. Miller Oil had the line buried and out of sight.
1751877233 2015-09-01 Production Wells No Violations On the day of my inspection I found that Miller had moved a service rig onto the well.
1950624245 2019-03-28 Production Wells No Violations Freeman Miller notified me this morning that they had one of their flow lines cut last night. He said that the landowner called to let them know what had happened, and they came out last night to shut the wells in and check on the situation. Freeman said that there was only a little water on the ground last night, and that is why they did not report it then. He also said that they were coming out this afternoon to repair the line and treat the affected area. When I arrived at the site I found that there was a 5 foot diameter area of soil that was covered with crude oil spray, and one 3 inch diameter puddle of crude oil beside the line. The incident took place in an agricultural field where a new home is being constructed. I was told that the line was hit when the landowner was digging a trench to run a house gas line from permit 7006. Miller Oil has 1 inch plastic line ran inside of 2 inch steel line from this well directly to the production equipment. There was some water dripping from the split line, so I put an absorbent pad down until Miller got here to start the clean-up. I called the DOGRM emergency response duty officer to report the issue.
2115787902 2015-04-14 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a pumping unit. The valves were closed and there was no pressure on the gauge. The electric box was in pieces on the ground. The polished rod was dry and rusted around the stuffing box. This well must be plugged within 180 days of the issuance of this compliance notice
2135205818 2015-10-22 Production Wells No Violations On the day of my inspection I was contacted by James Miller. He claimed that they got new liner in the flow line from the well to the tanks and they were ready to demonstrate that the well was capable of production. I met with James Miller at the well site. I witnessed the pumping unit in operation. I also witnessed the well making crude oil while pumping. The valve on the tubing was open and there was pressure on the gauge. This well has been made capable of production. This violation is resolved.
