0276284333 2013-07-24 Production Wells Violations Noted On the day of my inspection I found that the well head was equipped with a pumping unit. The tank battery consisted of two 100 barrel storage tanks, a concrete underground brine tank, and one separator. There were no leaks or spills. No identification was posted.

1488467724 2017-03-02 Annular Disposal No Violations On the day of my inspection I found that annular disposal was not in use at this well. There were two 100 barrel steel tanks and one concrete underground vault in use for fluid storage. There was a shallow earthen dike in place around the steel tanks. There were no leaks or spills around the steel tanks. The soil and vegetation around the concrete vault showed signs of brine impact. I contacted James Miller by phone to give him a verbal compliance notice. Miller Oil has until 4/2/17 to drain, and remove the concrete vault. See inspection #1764402202 for more detail.

1764402202 2017-03-02 Production Wells Violations Noted While conducting a TAAD inspection on this well I found visual evidence that the concrete underground brine vault was leaking. The grass around the south and east sides of the vault was dead, and the soil was dark in coloration. The grass around the north and west side of the vault was green and full, and the soil coloration matched that of the surrounding landscape. I called James Miller to report the issue and to ask that he remove the vault. I told James that the vault must be drained, broken up and removed. He agreed to have it done within 30 days.
1795323155 2017-03-31 Production Wells No Violations On the day of my inspection I found that the concrete underground brine vault had been removed from the site. A new earthen berm had bee constructed behind the existing tanks. I found no impacted soil remaining at the site. The compliance notice has been resolved.

2079137684 2016-12-08 Production Wells No Violations On the day of my inspection I found that the well was being produced with an electric powered pumping unit. The motor and drive belts were intact, and the electric was connected. The flow lines were connected at the wellhead and the valves were open. Identification was posted on the pumping unit, and there were no leaks or spills around the wellhead. The tank battery is shared with permits 3216 and 5100, and consisted of two 100 barrel steel tanks, one vertical separator and a concrete underground brine vault in a shallow earthen dike. Correct, legible identification was posted and I found no leaks or spills around the tank battery.
