0100571852 2017-05-31 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed another 7 tons of clay down the 4.5 inch diameter casing for a total of 10 tons on the bottom plug. They ran into rig problems when they started to pull the rest of the 4.5 inch casing and they had to shut down.
0508134227 2016-12-28 Production Wells Violations Noted On the day of my inspection I met the landowner Kevin Miller to conduct an exempt domestic pre-transfer inspection. There was no existing access route to the well. The terrain was difficult and there were no vehicle or foot tracks back to the well. Mr. Miller took me back in a UTV. He claimed that he has lived there for nine years and he has never seen anyone go back to the well. I found that the well was equipped with a 4.5 inch wellhead. There was approximately 3 feet of 2 inch tubing coming out of the top of the wellhead. The tubing was open to the atmosphere and seeping gas. The steel flow lines to the tank battery were disconnected and there was a 3 foot diameter area of crude oil on the ground that appeared to have come from the disconnected line. There were wooden supports and a broken electric pole, indicating that the well was equipped with a pumping unit at one time. The casing valve was closed and led to a 1 inch yellow plastic line that ran to a horizontal cylindrical tank of unknown volume. The tank was larger than a typical 200 gallon liquid propane tank. Mr. Miller claimed that both of the Walters wells and the Blackburn well from the neighboring property supplied gas to the tank, which supplied the domestic gas to the home. Mr. Miller opened the casing valve to the tank and gas started flowing out of the open tubing. There have been zero mcf gas and zero barrels of oil reported for this well since 2011, and no production reports have been filed for 2015. The well is idle and it is shut off from the domestic gas supply. When the valve to the gas storage tank is opened the gas back feeds into the well and blows into the atmosphere. I also inspected the other Walters well the same day and found it to be shut in as well. The domestic gas is being supplied by the Blackburn Unit 1, permit 0287. The tank battery consisted of one 100 barrel steel tank, one 30 barrel plastic tank and one vertical separator. The equipment was rusted and in a general state of n
0919577776 2017-06-07 Plug / Plug Back No Violations On the day of my inspection I found that the fluid level was down approximately 150 to 200 feet. Gene washed another ton of clay down the well and filled it to surface. They shut down for two hours to let the clay settle. The clay fell approximately 20 to 30 feet after two hours and Gene topped it off again. It was staying at the surface, so Gene said that he would leave the well open for a couple days while he rigged down and moved all of his equipment out. There was still at least 1.5 tons of clay left on the ground to top it off if needed.

1496249567 2017-05-30 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was rigged up to plug the well. They had an 11 ton load of clay on the ground. There were no rods or tubing in the well. Gene ran his tools to bottom to ensure the well was clear of obstructions and gauge the fluid depth. Gene washed 3 tons of clay down the 4.5 inch diameter casing. He went in to rip the casing and found a good telegraph at a depth of 3010 feet. Gene parted the casing there and started pulling. The casing was dragging hard, but Gene was able to work it free. They pulled 20 joints and shut down for the evening.
1496323710 2017-06-01 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was not plugging. The rig was still down for repairs.
1496432751 2017-06-02 Plug / Plug Back No Violations On the day of my inspection I met with ODNR surveyor Tom Smith to get the well location recorded. Gene Matheny was running new line on the rig. Matheny pulled the remaining 4.5 inch diameter casing from the well. We tallied 88 joints at an average of 33.80 feet per joint for a total of 2974 feet. Once the casing was out Gene set a pre-cast tapered cement plug on the 7 inch casing seat at a depth of 1020 feet.
1496841163 2017-06-06 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed 4 tons of clay on top of the pre-cast plug to cover the Berea. Gene hooked up to pull the 8.625 inch diameter surface casing. He pulled up to 100,000 pounds and the casing would not move, so he went in to rip. Gene got a telegraph at a depth of 437 feet and got the casing parted there. He pulled the surface casing from the well and washed approximately 4 more tons of clay in until the well was filled to surface. They shut down to let the clay settle overnight.
1526999484 2017-06-09 Plug / Plug Back No Violations On the day of my inspection I found that the well was still filled to the surface with clay. Matheny still needed to cut the casing off and cap the well with a steel plate. This well is plugged and abandoned. The compliance notice is resolved.
1787114215 2017-09-18 Final Restoration No Violations I received a signed landowner waiver (form 5) from Rick Wray for the final restoration on this well site. On the day of my inspection I found that the production equipment had been removed. The access road is still in use for agricultural purposes. The well site had been cleared of all oilfield related materials and debris, except the well casing, which was stacked on wood sills on the ground, and the electric pole. The well site and production equipment site were both rough graded and starting to grow with weeds and grasses. There were no erosion issues or wash outs. The landowner waiver stipulates that the landowner wants to keep the materials that were left behind. Rick Wray said that the landowner Kevin Miller paid him for the casing. This site passes final restoration with a waiver.
