0592450860 2015-05-26 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a pumping unit. The tank battery consisted of two 100 barrel tanks and one separator. There was a 30 barrel poly tank down by the road. There was no identification posted for this well. This well is in production. There was 0 mcf gas, and 0 barrels oil reported from 1993 to 2009. There have been no production reports filed since 2009. One of the 100 barrel tanks was leaking. There was crude oil and brine contamination inside of the dike. The side of the dike had been cut out to allow the oil and brine to run down the hill. There was a bare path of soil that was stained with oil that extended approximately 20 to 30 feet down the hill. There was also an open plastic line leading from the tank battery to the poly tank that had been running onto the ground. Repair or remove the leaking production equipment. Remove all of the contaminated soil from inside and outside the dike. Cap the plastic line with a shut off valve, and well identification must be posted.
0990242686 2015-08-05 Production Wells No Violations On the day of my inspection I found that identification had been posted on the 100 barrel tank. This violation is resolved.
1021512507 2017-02-03 Production Wells No Violations On the day of my inspection I found that the well was equipped with an electric powered pumping unit. The motor and drive belts were intact, and the electric was connected. The flow lines were connected and the valves were open. The tank battery consisted of one 100 barrel steel tank and one vertical separator inside of an earthen dike, and one 30 barrel poly tank outside of the dike. Correct, legible identification was posted and I found no leaks or spills.
1067024101 2015-06-01 Administrative Inspection No Violations I received a phone call from well owner Lena Stricker. She claimed that they were in the process of trying to contract someone to clean up the contamination and repair the tank. I advised her on the identification requirements and proper disposal of the contaminated soil. She claimed that she has been turning in production reports to the County Auditor every year, so I provided her with Steve Rileys phone number at DOGRM Central Office to get her production reports up to date with the State. This well is used solely for domestic gas. However, it could not be permitted as an exempt domestic well because the Strickers do not own the property where the well is located.

1436897687 2015-07-14 Production Wells No Violations On the day of my inspection I found that the leaking tank had been turned and moved to a new dike. The contaminated soil had been removed from inside the old dike and from the hillside. The production reports have been brought up to date. There was still no identification posted. I contacted Mrs. Stricker to remind her that they need to identify the well in order to be in compliance.
1574995351 2016-04-07 Production Wells No Violations I received an email notification of complaint 5294 alleging that this well was idle and requesting enforcement action to have the well plugged. On the day of my inspection I found that the well was being produced with an electric powered pumping unit. The electric was connected at the motor and the belts were intact. The tubing and casing valves were open and the polished rod showed signs of recent use. There was new poly flow line connected at the wellhead and the soil showed signs of recent excavation from laying the line to the tank battery. There were no leaks or spills at the wellhead. The tank battery consisted of one 100 barrel steel tank, one 30 barrel poly tank and one vertical separator. An earthen dike was in place around the 100 barrel tank and the separator. There was also one 100 barrel steel tank inside of a dike that was not in use. The Strickers claimed that they had a contractor lined up to remove the old tank and restore the old dike area. The flow line to the separator was connected and the in-flow valve was open. The out-flow valve was also open and there were connected lines to the 100 barrel tank and to a drip bottle. The valve from the drip bottle was open to 1 poly gas line laid in the direction of the Stricker home. The gauge at the separator was hard to read, but I believe it showed approximately 100 psi. Identification was posted on the tank and there were no leaks or spills around the production equipment. I went to the Strickers home and met with Mr. and Mrs. Stricker. They claimed that they use the well primarily for domestic gas. Mr. Stricker claimed that he pumps the well one time a week for three hours, and the well makes approximately one tank of oil every five years. The tank did have fluid in it, but I could not determine the volume or composition. I observed an external gas hook up and regulator on the outside of the home.
