0455257816 2013-05-01 Administrative Inspection No Violations I found the official record of the cancellation of the oil and gas lease associated with this well at the Coshocton County Recorders Office in volume 602, page 829. Additionally, I found aerial photos showing that the well head is outside of the property line of Zelpha Myers on the Coshocton County GIS database.

1299886744 2013-04-30 Production Wells No Violations I received a phone call from George Clark in regards to a domestic well on his property for which the lease was cancelled. On the day of my inspection I found that the well was equipped with a pumping unit and connected to a tank battery that consisted of one 100 barrel stock tank and one separator. I found no leaks or spills and identification was posted.
2118581633 2016-03-11 Production Wells No Violations I received a phone call from Debbie Helms of the White Law Office on behalf of George and Bonnie Clark. She called to request an idle well inspection in hopes that it would lead to enforcement action to plug the well. The well is registered as an exempt domestic well, so no production has been reported since 1998. On the day of my inspection I found that the well was equipped with an electric powered pumping unit. The electric was hooked up at the pole. The manual switch in the control panel was in the off position. The fuse in the breaker box was in the on position, and the meter showed 0.000 kwh (kilowatt hours). The electric was connected at the motor and the belts were intact. The surface casing head was buried and not accessible. The production casing was equipped with two nipples. One was equipped with a relief device and connected to black poly 1 line that entered the ground in the direction of the tank battery. The other nipple was shut in. Both casing nipple valves were closed. The tubing was connected to the flow line and the valve was open. There was a gauge on the line that showed 0 psi. The bridle was connected to the polished rod. The polished rod and stuffing box were dry. The wellhead and motor were entangled with vegetation and there was a sapling tree growing up along the polished rod. The tank battery consisted of one 100 barrel steel tank and one vertical separator. There was an earthen dike in place around the production equipment. The valves were off at the separator and the gauge showed 0 psi. I found black poly 1 line exiting the ground inside the dike from the direction of the well. The line was cut and open to the atmosphere. There was a disconnected drip bottle laying on the ground next to the separator. I tapped on the outside of the 100 barrel tank and estimated that it contained 3 feet of fluid. The load valve was not equipped with a plug or locking device. There was some minimal staining under the valve, but I found no active leaks. Th
