0629626451 2018-09-12 Plug / Plug Back No Violations On the day of my inspection I found that Rick Smith was not working and the next load of clay had not been delivered. The gas that was bubbling from the flow line had been stopped. I called Rick Smith and he said that he shut the line off at the separator. He also said that he was giving the site one more day to dry out, and he planned to bring the clay in tomorrow.
0763821381 2018-08-22 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was still pulling the tubing from the well. He said that there was a tight spot downhole where the tubing was dragging hard. Gene also said that the fluid was only two joints down, so he suspected a hole in the surface casing and the production casing. Gene said that he was going to use cement for the bottom plug.
0807732059 2018-08-28 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny had ran 7 inch diameter casing over the parted 4.5 inch diameter casing. He then milled the top of the casing open to where he could run the 2.375 inch diameter tubing back in to the Big Lime. Gene ran tubing to a depth of 2508 feet. He had Timco scheduled to perforate the casing in the top of the Big Lime tomorrow morning.

1025564994 2018-08-23 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny had the tubing out and he was running back in the hole to set a cement bottom plug. Gene ran the tubing to a depth of 3668 feet and scheduled Cemenco for the morning. Gene said that he calculated the tight spot at an approximate depth of 2700 feet, and he suspected that there was a hole in the production casing at that depth. Gene proposed running 4 or 5 tons of clay on top of the cement plug to ensure that the suspected hole is covered. The clay was delivered and I received a ticket for 9 tons.

1268923482 2018-07-26 Production Wells No Violations On the day of my inspection I found that there were no signs of any activity at the well. The access road was overgrown and too narrow for a service rig. The tanks were still in place and overgrown. The pumping unit was still in place and overgrown. I called Rick Smith and he claimed that he has been waiting on Gene Matheny. Rick guaranteed that he would be moving his dozer in on Monday to start preparing the site, and Matheny would be moving in next week. I reminded Rick that this was a complaint driven notice to plug and it was supposed to be resolved by June 2017.
1408932115 2018-09-17 Plug / Plug Back No Violations On the day of my inspection I found that Rick Smith was not at the site to put any more clay in the well. The fluid level was down approximately 50 feet. I called Rick and he said that he washed clay in on Friday, filling the well to surface again, but he had not been back out over the weekend. He said that he was not coming out today because of the rain, but he would call me tomorrow when he was coming out.
1458049443 2018-09-07 Plug / Plug Back No Violations On the day of my inspection I met Rick Smith at the well site. The clay and water was down approximately 150 feet. He had another load of fresh water delivered and he washed more clay in the well until it filled to surface. Rick said that he was going to leave it open over the weekend and check back next week. Rick said that he was bringing another 4 tons of clay out next week. He also had the pit drained and closed it.
1481300886 2016-12-09 Production Wells No Violations I received a call from landowner Don Selders to report an idle well on his property. Mr. Selders claimed that he hasnt received a royalty payment since 2014, and it was only $0.57. On the day of my inspection I found that the well was equipped with a pumping unit. There was no motor on the unit. There was an electric box lying in the weeds next to the unit. The flow line was connected at the wellhead. The tubing valve was open. The casing valves were both open. One connected to the flow line and one was open to the atmosphere. The gauges were corroded and not legible. The polished rod and stuffing box were dry. There were sapling trees and shrubs growing up through the frame of the unit and around the Pittman arm. There were no leaks or spills around the wellhead. The tank battery consisted of one 100 barrel steel tank in an earthen dike. The tank was empty. There were no leaks or spills around the tank and correct identification was posted. There was a production chart in the compressor station next to the tank. The chart was unlabeled, faded and blank. The production records are up to date. The well made 4 mcf gas in 2013, 1 mcf gas in 2014 and there was zero production in 2015. I called Rick Smith of HI Smith Oil & Gas to let him know that I had received a call on this well, and that I found it to be idle. Rick said that he would apply for a plugging permit and get someone out to plug the well.
1534773902 2018-08-20 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was rigged up on the well. He was getting ready to start pulling rods and tubing. I spoke to Rick Smith and he said that he was on his way out with the permit and a water tank.
1535115399 2018-08-24 Plug / Plug Back No Violations On the day of my inspection Gene Matheny had his tubing ran to a depth of 3665 feet, and he had a pit dug and lined. Cemenco pumped a 400 foot cement plug with 30 sacks of class A cement. The well circulated during displacement. Gene pulled the tubing from the well and rigged up to part the production casing. The casing parted up hole when Gene pulled to determine stretch. He pulled 112 feet of 4.5 inch diameter casing out of the well. The last joint was severely deteriorated. Gene speculated that the surface casing was also deteriorated at the same depth. He mentioned that he thought there was a hole in the surface casing when he first moved in. They shut down for the weekend to come up with a plan to get the casing out of the well.
1535380751 2018-08-27 Plug / Plug Back No Violations On the day of my inspection Gene Matheny was at the plug site briefly to get his truck so he could bring a string of 7 inch diameter casing back to the site. I spoke with Rick Smith, who proposed a plan for the well. They want to run in with the 7 inch casing to get over the parted 4.5 inch casing. Then they want to mill the top of the 4.5 inch casing to the point where they can get back in it with the tubing. They will run the tubing down to the top of the Big Lime and perforate the 4.5 inch casing, and squeeze a cement plug in the perforated casing, at the top of the Big Lime. Then t hey will shoot the 4.5 inch casing off below the Berea and pull it from the hole. Then Gene will set a pre-cast cement plug on top of the 4.5 inch casing, and finish plugging the well with clay.
1535551022 2018-08-29 Plug / Plug Back No Violations On the day of my inspection Timco perforated the 4.5 inch casing with 10 shots from 2655 to 2665 feet. Gene Matheny ran tubing to 2667 feet. Cemenco pumped a 40 sack cement plug through tubing, in the 4.5 inch diameter casing, above the perforations. I observed circulation from the surface casing and the production casing, which indicated that the perforations were open and communication was established. Cemenco displaced the cement to 2467 feet. Jerry said that his pressure indicated that the cement was displacing up the casing and the hydrostatic pressure would squeeze it out the perforations as it settled. Gene Matheny pulled all of the tubing out of the hole. Timco ran back in to a depth of 1205 feet to shoot the 4.5 inch casing off below the Berea sand. Matheny then ran in with a Halliburton packer on tubing. He got the packer set inside of the 4.5 inch casing and pulled it out of the hole.
1535648936 2018-08-30 Plug / Plug Back No Violations Matheny Well Service continues with the plugging this well today. They set the precast cement plug on the 7 inch seat at 1140 feet and washed in prepared clay to cover the Berea producing formation. They then dug around the well casing and cut off the weld around the 8 5/8 inch surface casing attaching it to the drive pipe. They then cut two (2) holes in the side of the surface casing and ran a steel rod through to lift tension on it to see if it was parted. The casing was free and has a string weight of 2,500 pounds. of 20 pound casing. Which indicated that the casing is parted around 125 feet in the well bore. They left for today to get tools ready to pull the surface casing from the well bore.
1536596812 2018-09-10 Plug / Plug Back No Violations On the day of m y inspection I found that the clay had fallen back down the hole. The top of the fluid was approximately 150 feet down. I also found gas bubbling up through the wet clay pile on one side of the wellhead. I called Rick Smith and he said that it was most likely gas back feeding through his flowline from the separator. He said that Gene Matheny told him t hat he smelled gas around the wellhead when he was plugging. Rick said that he was on his way out to shut the time in at the separator. The access road back to the well was very muddy, so Rick did not know when he would be able to get another load of clay delivered.
1537295319 2018-09-18 Plug / Plug Back No Violations On the day of my inspection I met Rick Smith at the well site to witness him washing more clay in the well. The fluid/clay was down approximately 50 feet. I could see the top from the surface. Rick filled the well to surface again. He said that he was going to let it settle for a couple days before coming back out Rick said that he would call me Thursday, depending on the weather.
1537453011 2018-09-20 Plug / Plug Back No Violations On the day of my inspection I met Rick Smith at the well site to top the well off with clay again. When I checked the well I found that it was still filled to surface with clay. The clay/water column had not fallen since the last time we were out. I told Rick to go ahead and cut the casing off 36 inches below grade and cap the well. He had a steel plate on site that was inscribed with the permit number. Rick said that he has been discussing the restoration with the landowner and they will probably waive the access road. The complaint for an idle well is resolved.
1558708418 2019-05-24 Final Restoration No Violations On the day of my inspection I found that the 100 barrel steel tank was still in place at the site entrance. The access road leading up to the well was passable by vehicle, but there were some ruts starting to form from water run-off. The upper and lower portions of the road are in flat, agricultural field and they were in good shape. The middle portion of the road is inclined and runs through forest cover, which is where the ruts were forming. The upper and lower portions of the road were 60 to 70 % covered in tall grasses, while the middle portion under shade was bare. I found a section of steel flow line exposed in the access road. The well site was still rough, with mounds of soil and the remnants of the clay pile. There were pieces of pit liner scattered around the site, and the wellhead components were still there. There was also a 100 barrel steel tank back at the site. I called Rick Smith and he claimed that he has been waiting on the weather to dry up. He said that he intends to move in next week to start the restoration.
1571987286 2018-09-04 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was rigged down and moved out. There was still about one ton of clay piled on the ground next to the open well. The fluid was approximately 50 feet down. The pit was still open, there was a 100 barrel steel tank and a mini track hoe left behind. I spoke with Gene Matheny and he said that they will have to keep coming back to add clay and let it settle. He said that the surface casing was rotted off at the same depth as the production casing, around 112 feet. The Knox County inspector Mark McCombs was on site to witness plugging operations on 8/30/18. He reported that Matheny set a pre-cast cement plug on the 7 inch casing seat at 1140 feet and then washed 4 tons of clay on the plug to cover the Berea sand. He also reported that the surface casing parted up hole when Matheny tried to pull it.
1636636054 2017-09-15 Production Wells No Violations I was contacted by Rick Smith to inform me that they were planning to move in to plug the well. I met with DOGRM surveyor Tom Smith and took him back to the well to get the survey done before it is plugged. The pumping unit was still in place and the valves were still closed. There were no leaks around the wellhead. The production equipment was still in place. Identification was posted and there were no leaks or spills.

2085641563 2018-09-13 Plug / Plug Back No Violations On the day of my inspection I met with Rick Smith at the well site. He brought in another 6 tons of clay. The fluid level was down approximately 200 feet. Rick washed nearly half of the clay down the well, allowing it to fill and settle over the course of the afternoon. The well eventually filled to surface and stayed. Rick shut down for the day and said that he would be back tomorrow to check it and continue to fill it as needed.
