0957206536 2017-05-22 Plug / Plug Back No Violations On the day of my inspection I found that Mike Wheeler was rigged up to plug the well. The pit was dug and lined. There was a 500 barrel frac tank of salt water and a water truck of fresh water staged on site. Mike had tripped the tubing out and ran back in to a depth of 5705 feet to set the bottom plug. Mike said that the tubing had been left in the hole with a rotary bit on the end. They were getting drilling mud back when they pulled the tubing. Mike said that he did not want to run the tubing any deeper than one or two joints past the bottom of the 4.5 inch diameter casing because he has encountered problems in the past when plugging a well that was drilled open hole and never cleaned out. Cemenco is scheduled to set the bottom plug tomorrow morning.
0970128675 2017-07-27 Final Restoration No Violations On the day of my inspection I found that the site was final graded and there were no materials or debris. The site was mostly bare with sparse weeds coming up. The surrounding landscape is relatively flat and there were no signs of erosion. I called Mason Producing and left a message to contact me about getting the site seeded. The access road and production equipment will remain in place for existing wells.

1251810591 2017-05-25 Plug / Plug Back No Violations On the day of my inspection I found that Mike Wheeler had ripped the 4.5 inch diameter casing at a depth of 4510 feet. Mike pulled the casing up to the bottom of the Clinton plug and Formation Cementing established circulation. They circulated approximately 10 barrels of oil out of the hole before they started mixing cement. Wheeler and Formation also set cement plugs at the top of the Big Lime, through the Berea Sand, at the base of the surface casing and at the surface. All of the plugs were set through 4.5 inch casing. Circulation was established and maintained throughout each stage of the plug job. Cement was circulated to surface on the final plug, and it stayed at the surface after pumping. Wheeler said that he would be back next week to cap the well with a plate.
1459037184 2018-06-07 Final Restoration No Violations On the day of my inspection I found that the well site was 80 percent covered in tall grasses and broadleaf vegetation. There were some areas of rocky soil that werent growing very well, but the site was mostly covered. The topography is flat and there were no issues with erosion. There were no oilfield related materials or debris left at the site. The access road and tanks are still in use for existing production wells. This site passes final restoration.
1495558352 2017-05-23 Plug / Plug Back No Violations On the day of my inspection Cemenco pumped a 30 sack bottom plug through 2.375 inch diameter tubing. The calculated plug depth was displaced to 5305 feet with 20.5 barrels of salt water. The well did not circulate during the plug. Mike Wheeler then pulled all of the tubing from the well. I met with Aaron Austin, field supervisor for Mason Producing. He said that there were no known problems with the 4.5 inch diameter casing. They are plugging the well because of the volume of water it produces. Aaron and Mike intend to plug the rest of the well through the 4.5 inch casing. Mike released the casing from the slips and shut down for the day.

2060656258 2017-05-31 Plug / Plug Back No Violations On the day of my inspection I found that Mike Wheeler had rigged down and moved out. The pit was closed and the site was graded to the surrounding contour. There were no oilfield related materials or debris left at the well site. I called Mike Wheeler and he said that they were out on 5/30/17 to cap the well and grade the site. He said that the cement was still at the surface when they capped the well. The access road is still in use for permits 1507 and 1527. The production equipment is still in use for permit 6516. The site still needs to be seeded for erosion control.
