0085294680 2015-06-25 Preliminary Restoration No Violations On the day of my inspection I found that 90% of the site was growing with well established vegetation. There was still an issue with water running through the site. It was creating a wash out that runs diagonally through the well site and down the hill. I called Bill Mitton and left a message to contact me.

I called and left a message with Bill Mitton. He returned my call and claimed that he intended to get the culvert installed the first part of the following week.

1371819141 2013-06-20 Casing String Witnessed No Violations On the day of my inspection I found that Kilbarger Drilling Rig #5 drilled a 14.75 hole on air to 65 and then ran new 32# 11.75 conductor casing to 62. Petroset broke circulation, pumped 100 lbs of gel, 50 lbs of cotton seed hulls and 50 sacks of class A cement. Good circulation was maintained throughout the job with 2 barrels of cement returns.
1372083977 2013-06-21 Casing String Witnessed No Violations On the day of my inspection I found that Kilbarger Drilling Rig #5 drilled an 11 hole on air to 644 and then ran new 24# 8.625 surface casing to 631. Petroset broke circulation, pumped a mixture of gel and cotton seed hulls, followed by 185 sacks of class A cement. Good circulation was maintained throughout the job with 12 barrels of cement returns.
1372166983 2013-06-25 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kilbarger Drilling Rig 5 had reached TD and set 4.5 production casing. The crew was rigging down and moving equipment off of the location. The permit was on display, the pit liners were in good shape and there were no leaks or spills.
1383050565 2013-10-28 Annular Disposal No Violations On the day of my inspection I met with UIC Inspector Dave Ball and Terry Wade of Knox Energy to witness a mechanical integrity test on this well for an application to use annular disposal. Terry Wade pressured up the annulus to 324 psi with a combination of nitrogen and natural gas. The pressure decreased to 300 psi within 15 minutes. The test failed. We will repeat the test at a later date.
1406209307 2014-07-23 Preliminary Restoration No Violations On the day of my inspection I found that the drill site had been seeded and strawed. There was minimal grass growth. The restoration does not pass at this time.

There was still an issue with water running off the hill and through the site. I called Bill Mitton again. He said they would go back out to remediate the restoration.

The well owner, Knox Energy, must seed or sod the site in order to bind the soil to prevent erosion and/or sedimentation. The preliminary restoration on this site was due on 12/12/13.
