0236012371 2016-04-26 Administrative Inspection No Violations I received a phone call from Lisa Wilson who claimed to be the granddaughter of one of the Snider Brothers. She called to inform me that she had been contacted by the White Law Office on behalf of Charles Beatty to claim ownership of the wells. I called Mr. Beatty to confirm the claim. Mr. Beatty said that he was letting his attorney take care of everything. I was contacted by Christopher White via email to inform me that he had found two executed and unrecorded assignments of wells and lease, and a fully executed well transfer document. I referred him to Jon Austin and Kathy Smith with the surety and bonding section. I also contacted Mr. White by phone to inform him that there was a complaint filed on the wells, and a compliance notice to plug and abandon was issued for both Beatty wells.
0570032073 2017-04-04 Administrative Inspection No Violations I received a phone call from Charlie Beatty to notify me that the plugging permit had been issued for this well. He said that he has contracted Gene Matheny to plug the well, and he would be out to start the job within the next few weeks. Charlie was concerned about the resolve by date. I agreed to be moderately flexible with the date due to the fact that he has followed through with obtaining the permit and contracting a plugging service.
1054420506 2016-10-18 Production Wells No Violations On the day of my inspection I met with landowner Charles Beatty. Mr. Beatty produced documentation from DOGRM which showed the well had been assigned to him as the rightful owner for exempt domestic use. There was no change in the condition of the well. It was still shut in and idle. Mr. Beatty had requested a meeting with me to go over the ownership documents, the outstanding violation, and to discuss his options with the well. When the violation was originally issued the well was under ownership of Snider Brothers, who are deceased. I had previously made contact with some family members, who had agreed to plug the well, although there were claims that Mr. Beatty was the owner of the well, but no paperwork had ever been filed. Apparently, the original bill of sale had been produced and Mr. Beatty honored the agreement by taking ownership. The compliance notice resolve date had already passed by the time the transfer had gone through. Mr. Beatty was fully aware that a complaint had been file against this well and it was under a compliance notice to be plugged. He also did not dispute the claims that there is a hole in the casing and therefore the well is no longer capable of production. Mr. Beatty stated his intentions to have the well plugged and abandoned. He asked that an extension be given to allow him the 180 days to do so. I spoke with SE District supervisor Mike Moreland and we agreed to grant Mr. Beatty the extension.
1152717102 2017-12-12 Final Restoration No Violations On the day of my inspection I found that the areas where the well and tanks were located had been graded out to the contour of the surrounding landscape, and they were covered in short grass. There were no signs of erosion. The well owner/landowner Charlie Beatty is keeping the well casing that was removed during plugging. The casing has been removed from the well site and stacked near Mr. Beattys barns. He is also keeping the two 100 barrel steel tanks for future use. There were no plugging materials or debris left behind. The route that was used to access the well is still in use for daily farming operations. This site passes final restoration.
1339448677 2017-04-26 Plug / Plug Back No Violations On the day of my inspection Gene Matheny had another 8 tons of clay delivered. They washed the entire load down the well filling it to surface several times. I stayed on site to watch the clay while Matheny was grading the rest of the area. The clay would fall and they would top it off. The clay stayed at the surface once they got the whole load in. They cut the casing off three feet below grade and the clay was still sitting firm at the surface. They capped the casing with a steel plate and buried the well. This well has been plugged and abandoned. The compliance notice is resolved. I called Dwayne Dugan to notify him the well had been plugged. His complaint is also resolved.

1492101375 2017-04-12 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was moving in and rigging up to plug the well. Matheny moved the pumping unit and broke down the wellhead components. Then he un-seated the downhole pump and pulled a few rods. They shut down for the day until they could get direction from Mr. Beatty as to where he wants the well materials staged on his farm.
1492784959 2017-04-20 Plug / Plug Back No Violations Gene Matheny reported that on Wednesday he set a pre-cast cement plug on the 7 inch seat at 890 feet, then he washed 2 tons of clay across the Berea and got the 8.625 inch casing started out of the hole. On the day of my inspection Gene Matheny pulled all of the surface casing from the well and then washed 8 more tons of clay in the well. The well did not fill up and the fluid level was estimated to be at approximately 200 feet. They scheduled another delivery of clay.
1495035491 2017-05-16 Plug / Plug Back No Violations On the day of my inspection I met DOGRM surveyor Tom Smith at the site to get GPS coordinates for the plugged well. We located the well with a metal detector and Tom recorded the coordinates. Charlie Beatty had the site graded back out to the original contour, and he was in the process of dismantling the production tanks.
1566601160 2017-04-18 Plug / Plug Back No Violations On the day of my inspection I met Gene Matheny at the site. He claimed that he washed 2 tons of clay on the bottom and got the 5.5 inch casing parted on Monday. He ripped the casing at a depth of 2130 feet. Gene pulled 20 joints and washed another 6 tons of clay down the well for a total of 8 tons on the bottom plug. They continued to pull the 5.5 inch casing.
2093997790 2017-04-13 Plug / Plug Back No Violations On the day of my inspection Gene Matheny pulled the rods and tubing from the well. They could not get clay delivered until Monday. They will return tomorrow to pull tubing and get the well prepped for clay. I told Gene that I will not be present Monday and I would like to have a minimum of 8 tons of clay on the bottom plug.
