0157194672 | 2017-01-25 | Drill / Deepen / Reopen | No Violations | I received a call from Dwayne Dugan on the evening of 1/24/17 to report that H&D Drilling had lost the dart from the 5inch bailer down the hole. He said that they were running in with a magnet in an attempt to retrieve the dart. On the day of my inspection I found that Max Donaker was running in with a 6 inch mandrill to set down on the top of the 5.5 inch casing in order to get an accurate depth of the top of the casing. Max said that he believed they were starting to clean out the 4 inch liner, when they lost the dart, because they were getting paraffin in the bailer. He believed that the dart was suspended in paraffin above the 4 inch liner. They determined the top of the 5.5 inch casing to be at a depth of 2730 feet, and the top of the 4 inch liner to be at a depth of 2846 feet. H&D made two unsuccessful runs with the magnet, so they switched to an improvised friction overshot in hopes of pulling the dart out in a plug of paraffin. They made three more runs with no success. Then they ran in with a 5 inch mandrill to ensure that the casing was open down to the top of the 4 inch liner. Max insisted that the dart was too wide to go into the liner and predicted that it must be sitting on the top. |

The lower portion of the access road was covered in short grass. The portion of the access road that went uphill to the wellsite was moderately covered in grass and showed no signs of erosion or wash-outs. The well site was almost completely covered in grasses. There were a couple small areas that were holding water, where the grass wasnt growing. I found no signs of erosion or run-off at the wellsite. The only thing preventing the final restoration from passing was that the gravel base for the pumping unit, and two steel braces were still in place. |
0341304170 | 2017-01-11 | Drill / Deepen / Reopen | Violations Noted | I was contacted by Dwayne Dugan to notify me that he was going to be running and cementing 7 inch surface casing. I had not received any notification of commencement of drill site construction, or reopen operations. I let Dwayne know that he was in violation for failure to notify. He did give 24 hours notification for the casing. I also let Dwayne know that he was in violation for failing to follow the approved casing program on the permit. Dwayne claimed that there was 8 inch conductor casing in the hole. And he intended on leaving it, rather than running the 10 inch casing on the permit. He also claimed that H&D Drilling was at a depth of 730 feet, and he intended to set 7 inch surface casing through the Berea. The approved casing program called for surface casing to be set 50 feet below the Big Injun sand, at approximately 455 feet. I told Dwayne that he could not proceed until I discussed the variances with DOGRM permitting geologist Steve Opritza. I contacted Steve to discuss the situation, and he agreed to allow the existing 8 inch to be left in place as conductor. However, Steve insisted that the 7 inch surface casing must be set above the Berea. I called Dwayne to give him the instructions. Dwayne agreed to the variances. |

The lower, flat portion of the access road was showing some sparse grass growth, but there were multiple areas where the road was laying wet with standing water. The uphill portion of the access road was completely bare; I did not see any grass seed and there was no straw down. There were no water breaks or drainage diversions cut into the road, and the there were some minor erosion channels starting to form. The well site was also mostly bare except sparse weeds. There were numerous pieces of pit liner sticking out of the soil throughout the site, and I found two sections of drilling line left on the site. The gravel and two steel pipes were also left behind where the pumping unit was set. The portion of the drill site that was close to the cut was laying wet with standing water. I did not find any grass seed of straw on the drill site. This site does not pass final restoration at this time. I spoke with Dwayne Dugan in person to notify him of the issues that need to be addressed. He was not very receptive and he would not give me a date when he could have it finished. |

Formation Cementing is scheduled for 9 am the following morning. |

Max Donaker found the top of the 5.5 inch casing to be at a depth of 2730 feet, and the top of the 4 inch liner to be at a depth of 2846 feet. Dugan is running the 4.5 inch casing to the top of the existing liner, so there will be approximately 116 feet of welded 4.5 inch casing cemented inside of 5.5 inch casing. The remainder of the casing will be in 6.5 inch hole. Dwayne Dugan gave me documentation of the used casing being pressure tested and drifted. |

1489759914 | 2017-03-16 | Preliminary Restoration | No Violations | When I pulled into the lease road and parked next to the tank battery I noticed an occasional rotten egg odor that is indicative of hydrogen sulfide gas or H2S. I powered on my personal H2S detector and got out of the truck. The wind was gusting and the odor was not constant. The H2S detector showed a reading of 0.6 parts per million (ppm). I drove back to the entrance and called Dwayne Dugan. He said that they were still pumping frac water out of the well and they had recently noticed an odor from the water. Dwayne acted a though he was not familiar with H2S gas. He said that they had taken a sample and sent it to Producers to be tested, as he believed the odor to be attributed with chemical additives in the frac water. I left the site and contacted senior inspector Pat Shreve who agreed to meet me at the site. When we arrived I found that Travis Dugan was on site to start the pumping unit and check the tanks. I asked Travis if they had any knowledge of encountering H2S gas. Travis also acted as though he was unfamiliar with H2S. He said that they had noticed the odor 7 to 9 days ago, and he also attributed it to frac water additives. We all walked up to the well. The pumping unit was in operation. There was no noticeable odor around the well site and it was minimal at the wellhead. The personal detector was still showing 0.6 ppm. Travis collected a sample of the water from the tubing valve. Pat held his detector over the opening of the bottle and it showed a reading 5.5 ppm. The water was dark in coloration. Travis said that there was virtually no gas being produced from the surface casing yet and they had to run a line from another well to fuel the engine on the pumping unit. We walked back down the hill to the tanks. The odor was noticeable again once we got near the tanks. Travis climbed up on top of the tanks and opened the hatch to the water tank. He held Pats H2S detector over the opening and the alarm was triggered. Travis said that the reading was at 25.5 |

There were five 300 barrel frac tanks staged at the bottom of the hill next to the access road and one 300 barrel tank at the top of the hill on the edge of the location. The 4.5 inch diameter casing was still stacked next to the well. The drilling/plugging pit was still open. The fluid level was low and there was no immediate danger of breeching the pit walls. There were no sags in the liner and I did not observe any rips or holes. The well was still open and the casing had not been cut below grade or capped. The open casing was filled with water and I could not see the top of the cement. I did not observe any gas bubbles or crude oil in the water. I called Travis Dugan to remind him that the pit needs to be closed right away. I also addressed the need get the well casing cut off and capped and the condition of the lease road. |
