0134533063 2015-04-02 Final Restoration No Violations I inspected the former location of this producing well as part of final restoration following plugging and abandonment which occurred on 9/17/2014. At the time of this inspection, production elements have been removed and the former well site area has been restored. The gravel-paved lease road is still extant and must be removed, or a waiver to leave the road agreed to by the land owner, prior to approval of final restoration.
0665863806 2017-07-06 Final Restoration No Violations I inspected this well location for evaluation for the requirements for Final Restoration. At the time of this inspection, I observed that the former well and battery locations had been restored, with a good development of natural vegetation throughout. The former gravel-paved lease road remained in place. I contacted Ralph Gregory about the lease road. He indicated that Bass Energy would attempt again to make contact with the landowner regarding proposed final disposition of the road. He has reported that the landowner has not responded to previous attempts to solicit a waiver of the lease road restoration requirements.
0811639025 2013-06-14 UrbanProduction Wells No Violations The unit consists of a wellhead with principal valve tree but no other production equipment. The well is currently shut-in. There are no other production appurtenances (e.g. Tank battery, flow lines). The wellhead is in a locked, chain-link enclosure. The unit does not pass preliminary restoration pending completion of production elements.
0957724585 2018-05-03 Final Restoration No Violations I inspected this well location as an evaluation for final restoration. At the time of this inspection, the former area of the well was restored with well established vegetation. The production elements had been removed previously. There were no signs or vestiges of the former production system. The dedicated lease road remained in place, and comprised of mixed gravel, and grassy overgrowth. I spoke with the land owner regarding submitting a waiver for the lease road. He indicated that he was thinking it over and we would discuss again in several days. Final restoration remained pending.

1601745947 2018-08-31 Final Restoration No Violations I was on location to evaluate the location for evaluation for Final Restoration following plugging and abandonment that was completed on 9/17/14. At the time of this inspection, I noted the following: the former location of the well had well-established vegetation with no signs of disturbance. No production equipment remained. Per agreement with the landowner, the gravel-paved access road was to remain in place. Upon inspection, I found the road to be in level, well-drained, and in good condition with no signs of erosional deficits.
