1225135852 2014-05-15 Final Restoration No Violations I inspected the area of the former producing unit which was plugged and abandoned on 07/12/2013. The area of the access road, wellhead and battery have been regraded and reseeded. Grass-type vegetation is established. Production equipment and associated appurtenances have been removed with the exception of the 100-barrel production tank which has been staged near the access road entrance. Also some of the restored area has been deeply rutted by vehicle traffic. Final restoration is not complete pending removal of the tank and final regrade/repair of the rutted portions. Subsequent discussions with Cutter Oil indicate that the issues will be tended to within 30 days.
1272398334 2013-07-08 UrbanPreliminary Restoration No Violations The producing unit consists of a an above-ground storage tank (AST) battery with 1 140-barrel steel production tank, and 1 50-barrel steel brine tank, a mechanical separator, and metering run. Identification is present. There is an earthen secondary containment which is in good condition. The load-outs are bull-plugged. The wellhead is located 50 feet southeast of the battery. The wellhead pumpjack is being dismantled ahead of anticipated plugging and abandonment. Preliminary restoration passes. Final restoration work will commence after plugging.
1566080331 2017-07-07 Final Restoration No Violations I inspected this former well location as an evaluation for Final Restoration. At the time of this inspection, the former areas of the well and battery had been restored. Meadow grasses and other vegetation was well established. No other activity is needed. Final Restoration is passed.
1582875155 2013-08-26 Plug / Plug Back No Violations Former location of this producing unit is being regraded. The wellhead was formerly located in the back yard area of the St. Martins Church in Bentleyville. The wellhead is now sealed and covered with fill. Elements of the former tank battery are still in place and remain to be dismantled and removed.
