0461347597 2013-08-16 Plug / Plug Back No Violations Bottom plug tagged at 1350 and the pipe was ripped at 1040. All the 4 1/2 production casing was pulled and a 45 sack plug of Class A cement was placed through tubing from 408 to 178. The plug will be allowed to set up and then tagged to verify the depthe before placing the 100 to surface plug.
1016854772 2004-08-18 Production Wells No Violations No violations
1050950779 2003-04-16 Production Wells No Violations No violations at the time of inspection.
1278822741 2013-08-16 Plug / Plug Back No Violations Josh Garland, inspector from Athens County, observed the setting of the bottom of the 7-inch surface casing at 273-feet for a duration of 5 hours. I could not observe the rest of the plugging on this well as I had my own issues at Equity O & G, Riley # 1 in Lawrence County. They circulated 40 sacks of Class A neet cement for the 100 in-feet/100-feet out on this plug. They later determined the top of that plug at 150-feet inside this casing before they set the top 7-inch surface plug at 100-feet to surface. They circulated 30 sacks of Class A neet cement to plug the top of this casing and then mobilized their equipment to the last well.
1398385821 2014-04-24 Plug / Plug Back No Violations Upon inspection, it was observed that this site is still largely bare. The pits were filled in. Surface casing was sticking approximately 1 out of the ground and cement could be seen about 6 down. I contacted Tim Wyant of Wyant Exploration Co. to discuss the restoration of this site, and he informed me that AEP waived the final restoration because they have plans to expand their landfill into the area where these wells were plugged. We do not have the landowner waiver on file. I will contact Tim Wyant again to see if he has a copy of this waiver. The restoration will not be passed until the waiver is approved.
1434541682 2015-06-16 Final Restoration No Violations On my inspection I found that the location had been restored according to 1509.072. I passed the final restoration at this time.
1765640412 2013-08-15 Plug / Plug Back No Violations Visited the site in the early morning and met with Tim Wyant of Wyant Exploration. Bowers equipment was already on location. They were going to set the Berea plug today after they removed the 1-1/2 inch tubing. I left before they started to attend to site preparation for the plugging of the Equity O & G Riley, API # 3408720176. From Tim Wyant later in the day Bowers crew bullhead the Berea plug over the perfs from 1638 to 1622-feet. They first removed and tallied the 1-1/2 inch production tubing for a total of 1620-feet. They pushed 30 sacks through the 4-1/2 inch production casing over the Berea perfed interval. They tagged the plug at 1350. They ripped and recovered the 4-1/2 inch production casing at 1040-feet. Shut-in the well for the evening.
