0363757311 2014-02-12 Production Wells No Violations On the day of my inspection, I reviewed The Division of Oil and Gas production records and found them current. The well appears to be capable of production and is equipped with a pumping unit as well as eight and five eights surface casing with four and a half inch production casing, the valves were open at the well, there is a gauge on the production line showing 150 pounds of pressure that leads into a small compressor / chart recorder / drip / separator / domestic gas line / fifty barrel poly brine tank and (2) one hundred barrel production tanks setting inside a containment dike, the I.D. is posted at the tanks.

I met with Bill Mitton of BWS and reviewed their plans which will commence tomorrow for plugging of the well.

1513259806 2017-12-14 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service (BWS) and Cemenco Services onsite. Cemenco bullheaded a 30 sack Class A Clinton cement plug weighing 15.6 pound per gallon from 5192 feet to 4841 feet squeezing .75 barrels away and displacing cement with 76.9 barrels of water. BWS gained telegraph at 3769 feet and successfully ripped casing. BWS pulled 1 joint of 4.5 inch diameter casing. Cemenco gained circulation and pumped a 43 sack 150 foot Class A Big Lime cement plug weighing 15.6 pound per gallon from 3725 feet to 3575 feet. Cement was displaced with 56.8 barrels of water. BWS tripped 4.5 inch diameter casing to 1618 feet. Cemenco gained circulation and pumped a 60 sack 200 foot Class A Bottom of the surface casing cement plug weighing 15.6 pound per gallon from 1618 feet to 1418 feet. Cement was displaced with 22.5 barrels of water. BWS tripped 4.5 inch diameter casing out and pumped a 31 sack 120 foot Class A surface cement plug with 2% gel, weighing 14.7 pound per gallon from 120 foot to surface circulating cement to surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.

During my inspection production equipment and pumping unit were on location. The well had an identification plate stating 4026 on top of the 11 inch diameter casing. The location had not been graded. David Hill Incorporated remains within the timeline for final restoration requirements listed in Ohio Revised Code 1509.072 (B).

1582654521 2017-08-01 Production Wells No Violations On the day of my inspection I witnessed the production equipment included 2-100 barrel tanks, a 30 barrel polymer tank, a vertical separator, a drip, domestic gas service, a 2 inch salesmeter with .375 inch orifice and a chart recorder. The well was equipped with 11.75 inch diameter conductor pipe, 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, a Gardner pumping unit powered by electricity with all valves in the closed position. Also present near the wellhead a compressor unit was located. Identification was present and production records were current. A plug permit was issued for the well on July 20, 2017.
1597928146 2018-09-05 Final Restoration No Violations The purpose of my inspection was a final restoration. During my inspection the location appeared to meet requirements for final restoration. All equipment and debris have been removed. The location was put back to original grade.
1621198884 2018-08-15 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration. During my inspection the pumping unit had been removed and the tank battery had been disconnected and lines removed. Two 100-barrel tanks and one 30-barrel poly tank remains. Once the tanks have been removed the location will meet the final restoration requirements.
