0377376930 2019-02-05 Production Wells No Violations The inspection is just on well head only. Luke Mallet accompanied me on the inspection and revealed the first compressor feeds the this well PN-4274 and PN- 4276. The well is compressing gas down casing and feeding oil and gas back into the tubing. The gauge on the well head is 750 psi and will get measurement tomorrow. The red valves are safety valve shut offs and hold true on the Gas Production Units.

During my inspection the wells were equipped with a production tree style wellhead and were equipped to produce with an enhanced recovery method by injecting produced gas into the 5.5-inch diameter production casing. The well appeared to be producing. The production equipment consisted three sand traps, three gas production units, twelve 400-barrel tanks, a sand purge tank, one 200-barrel tank, one combustor, and three compressors. Identification was at the well entry, the wellhead, and tank battery. Production records were current. Accompanied by ONDR inspector Pat Shreve and ODNR Regional Supervisor Brent Bear.

The production equipment consists of one Gas Production Unit, [12] 400-barrel steel tanks enclosed within a galvanized impoundment. The gas vapors are sent thru a gas Cyclone and burned. There is a compressor to compress gas and send the gas back down the tubing annulus.

1558950220 2018-08-15 Production Wells No Violations Brent Bear and I accompanied Austin Gutridge on the inspection. The inspection revealed the well is equipped with a large flange style well head with 2 master tubing valves and 1 flowline valve. The well is an enhanced gas system by compressing gas down tubing annulus and produce the oil thru the tubing. The tank site consists of [12] 400-barrel steel tanks, [1] 40-barrel steel tank enclosed within a Galvanized and rubber impoundment. There is a gas production unit and a gas cyclone. The compressors are over by the entrance of the pad.
1572006374 2019-03-12 Production Wells No Violations The inspection revealed the well is equipped with a Cactus flanged well head assembly. The 2--inch casing. The stainless-steel riser and gauge were placed on the well head and the pressure is 700 psi.
1771239713 2018-09-05 Production Wells No Violations The purpose of my inspection was a status check. I was contacted by Pin Oak well tender Luke Mallet. Mr. Mallet stated that he has installed gauges for annular pressure monitoring. During my inspection the wells were equipped with a production tree style wellhead and were equipped to produce with an enhanced recovery method by injecting produced gas into the 5.5-inch diameter production casing. Production valves were in the open position. Annular pressure monitoring was now present. The production equipment consisted three sand traps, three gas production units, twelve 400-barrel tanks, a sand purge tank, one 200-barrel tank, one combustor, and three compressors. Identification was at the well entry, the wellhead, and tank battery. Production records were current.

1882352471 2018-08-20 Production Wells No Violations The purpose of my inspection today was a status check. On 8/15/18 I met in person with Luke Mallet the well tender for Pinn Oak. I explained to Mr. Mallett that gauges will need installed to properly monitor annular pressures. Mr. Mallett was unsure of the current annular pressures for each well and he stated he will have the work completed in two weeks. During my inspection today, the gauges have not yet been installed. A follow up inspection will be conducted.
1946309328 2014-09-18 Completion Testing No Violations On the day of my inspection I witnessed that flow back had begun at 8:30 am with 1100psi gas was being sent down the salesline, production water to the frac tanks and condensate to the storage tanks. Met with Company Man Jay Johnston and completed walk around. Flow back will begin on the 580010 and 580035 later today and continuing for 4 days.
