0114717172 2018-10-11 Production Wells No Violations During my inspection Fountain Quail Rig 4 was on location preparing to drill two additional wells on pad. The well was still producing during inspection. I spoke with Dave Grey, the company man on location for Utica Resources and Mr. Grey stated that they plan to produce the well until the super triple rig comes on location to drill the curves and laterals on the other two wells 7H and 8H.

The production equipment consisted of fifteen 500 barrel tanks, four gas production units, two gas compression skid, four vapor recovery towers, horizontal vent scrubber, and one combustor. Identification was present and production records were current.

Identification was present and production records were current. Utica Operating Resources has been notified to identify the gauges for annular pressure monitoring.

The well was caged while the 2 wells that were recently added to the pad undergo drill-out operations.

The well is equipped with a large Texas Style gas plunger lift system with a steel cage built over the well. The production master valves are open, and the annular valves are open measuring pressure. The tubing pressure is 550 psi and the annulus on the 9 5/8 is reading 200 psi. The annulus on the 13 3/8 is reading 200 psi.

Express Energy Services cemented the pipe with 320 sacks of Class A cement with 2% CaCl. Had good returns and cement stayed to surface.

0881131780 2018-08-16 Production Wells No Violations The purpose of my inspection was a status check. During my inspection Quintana Energy Services (QES) was on location with a Nitrogen truck working on an enhanced recovery project. I spoke with Utica Operating Engineer Catlin Ball and he stated that the well had taken on a large quantity of fluid and they decided to utilize a QES nitrogen truck to inject nitrogen within the 5.5-inch diameter production casing to lift the column of fluid up the production tubing. Once the fluid has been removed and natural gas circulates back around so the plunger lift can normally function they will discontinue pumping with Nitrogen.
0968636104 2018-09-25 Production Wells No Violations The purpose of my inspection was to document the status of Utica Resource Operatings Dynamite Pad prior to SIMOPS activities. Guerney County Inspector Austin Gutridge was present during the inspection. At the time of inspection well #2-4339 remained in production. Correct identification was present. The cellar was backfilled with gravel and meteoric water. Gas bubbles were observed breaking on the surface of the cellar. A total of four (4) production wells were on the pad. Well #2-4339 will be outside of the fall radius of the anticipated rig contracted to drill the two permitted wells on the West side of the pad and the Owner has requested to produce well #2-4339 during drilling operations. The situation will be discussed further with Deputy Chief Bob Worstall. Inspector Gutridge will contact the owner to mitigate the gas leak within the cellar.

Conductors had been cemented for 3405924532 and 3405924533. The well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of fifteen 500-barrel tanks, four gas production units, two gas compression skids, four vapor recovery towers, horizontal vent scrubber, and one combustor. Kevin Rothenbuhler was contacted about the cellars bubbling on the 3H and 4H. Mr. Rothenbuhler stated that they would remove the pea gravel from the cellars on all wells and tighten valves and fittings that are plumbed up for annular pressure monitoring to remediate the bubbling.

The well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of fifteen 500-barrel tanks, four gas production units, two gas compression skids, four vapor recovery towers, horizontal vent scrubber, and one combustor. Kevin Rothenbuhler contacted me and stated that the pea gravel was removed from the cellar, the valves for annular pressure monitoring were tightened, and while utilizing a gas sniffer no leaks were detected. The pea gravel was placed back in the cellars today and no bubbling was present.
1634560364 2018-11-08 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-4339. At the time of my inspection the well was outside of the fall radius of the Patterson #343 and remained in production. Patterson #343 was rigging up to drill two additional wells on the pad. Jersey barriers were placed outside of the well head to prevent a collision. Pressure gauges to monitor annular, production, and tubing pressure remained in place. The intermediate casing pressure gauge indicated 180 psi. The production casing annulus indicated 180 psi. No leaks were observed. The flare stack was positioned in the Southwest corner of the pad and away from the well head and surface equipment. The proposed and approved SIMOPS plan conditions were met at this time.

1784092013 2014-07-29 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Southeast Regional Supervisor Rocky King, I met with Company Man Larry Howell. H & P Flex Rig #385 had drilled a 12.25 inch diameter hole to a depth of 3417 feet and was tripping out of the hole to get directional tools with an estimated td of 6400 feet for the 2nd intermediate section.

Fountain Qual Rig 4 was on location adding 2 more wells to the pad.
