0055273488 2016-10-13 Production Wells No Violations On the day of my inspection following up on Compliance Notice -81773869, I performed field tests on the north fill slope drain revealed a pH of 7 and chloride level of 541 parts per million (ppm). Field tests performed on the west fill slope drain revealed a pH of 7 and chloride level of 358 ppm. I also performed field tests on the east sump revealing a pH of 6 and chloride level of 491 ppm. The well was equipped with Sweco wellheads and a plunger lift system. Gauges present read 3050 pounds per square inches on the casing and 2830 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater, a GP unit and a 400 barrel purge tank within containment and shared with the 9H, 5H, 3H and the 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales.
0081773869 2016-08-23 Completion Testing Violations Noted On the day of my inspection on the north fill slope at Global Positioning System (GPS) coordinates 40.040271 -81.235808 on field testing revealed a pH of 7 and chloride level of 1771 parts per million (ppm). The sediment pond contained minimal fluid which was still plumbed into a nearby frac tank. I performed field tests on the two sumps revealing a pH of 7 and chloride levels of less than 31ppm. Further inspection revealed stressed was found on the west fill slope at 5 new areas at GPS coordinates 40.040397 -81.236133, 40.040271 -81.235808, 40.040222 -81.235692, 40.039766 -81.235993 and 40.039094 -81.235717. Field testing the areas revealed chloride levels ranging from 1000 ppm to 500 ppm with pH levels of 7. Eclipse EHS Supervisor Bruce Carpenter arrived and proposed digging a ditch near the northwest corner of the pad inside the underlying drain to try to determine if the pad was holding water and by passing the underlying drain. The ditch was dug revealing no water and then the underlying drain was exposed revealing water was going under the drain reaching the shale and breeching the pad. Owner/Operator shall remove contamination, dispose of properly and replace with clean fill. Decision was made by Eclipse to remove contamination, install 6 drains which will lead to the sediment pond and flush pad with fresh water until chloride levels are less than 250 ppm. Eclipse will submit a plan to remediate the drainage system at a later date.

0320104099 2017-06-07 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with a plunger lift system with valves in the open position. Gauges present read 1060 pounds per square inches on the casing and 770 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater and a GP unit. Also present a 400 barrel purge tank within containment and a blow case skid both shared with the 9H, 5H, 3H and the 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales. Field testing of the sediment pond revealed a pH of 7 and chloride levels of 46 parts per million.

At this time the job has been shut down due to the seismic activity in the area earlier today. I completed walk around with Mr. Durst prior to departure from the location.

Field tests performed on the west fill slope drain revealed a pH of 7 and chloride level of 412 ppm. Mustang Oil Field Services was onsite pumping out the sumps. The new seep which was discovered on 12/1/16 on the north side slope remains open and was field tested revealing pH of 7 and chloride level of 241 ppm. Various puddles were field tested on the pad revealing a pH of 7 and chloride level of 39 ppm.

Field tests performed on the west fill slope drain revealed a pH of 7 and chloride level of 136 ppm. I also performed field tests on the east sump revealing a pH of 7 and chloride level of 33 ppm. The new seep which was discovered on 12/1/16 on the north side slope remains open and was field tested revealing pH of 7 and chloride level of 66 ppm. Various puddles were field tested on the pad revealing a pH of 7 and chloride level of 33 ppm. The sump pump controlling the level of the sediment pond is having trouble keeping up with recent rainfall and discharge from the pad. The well was equipped with Sweco wellheads and a plunger lift system. Gauges present read 2345 pounds per square inches on the casing and 1960 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater, a GP unit and a 400 barrel purge tank within containment and shared with the 9H, 5H, 3H and the 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales.
1034864827 2014-11-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Drilling Rig #531 had drilled an 8.5 inch diameter lateral hole to the depth of 14550 feet. H&P ran and successfully cemented 14546 feet of 5.5 inch production casing. Site inspection revealed that H&P Drilling Rig #531 was walking the rig to the Wheeler 5H and the pits were being cleaned. H&P plans to begin drilling late tonight on the 17.5 Inch diameter surface hole.

All the seep drains are now tied into the sediment pond with plans to begin flushing the northwest corner of the pad with fresh water.

I also performed field tests on the two sumps revealing a pH of 7 and chloride levels of less than 31ppm. Contamination has been removed from the north fill slope with installation of the drain completed. Installation of the first drain on the west slope is nearly completed with 3 additional drains to be installed. Upon completion flushing of the north corner of the well pad will commence. The frac tank along with the roll off boxes are now on secondary containment.
1287356702 2016-08-26 Completion Testing No Violations On the day of my inspection following up on Compliance Notice -81773869 and performing field tests on the north fill slope at Global Positioning System (GPS) coordinates 40.040271 -81.235808 results revealed a pH of 7 and chloride level of 1175 parts per million (ppm). I also performed field tests on the two sumps revealing a pH of 7 and chloride levels of less than 31ppm. As of now approximately 4 inches of soil has been removed from the north fill slope and the extended sediment pond is nearly completed. Work will continue through the weekend removing additional contamination and tying all the slope seeps to the lined sediment pond. I met with Eclipse EHS Supervisor Bruce Carpenter, EHS Technician Dan Hockenberry where Mr. Carpenter stated that the secondary containment would be installed under the roll off boxes which were being used for excavation of the contaminated soils.
1304695749 2016-09-09 Completion Testing No Violations On the day of my inspection following up on Compliance Notice -81773869 and performing field tests on the sediment pond at Global Positioning System (GPS) coordinates 40.040488 -81.236108 results revealed a pH of 7 and chloride level of 917 parts per million (ppm). Testing of the water collected from the west fill slope revealed a pH of 7 and chloride level of 203 ppm All wells were shut-in at this time as flowline installation was being completed on the 1H, 3H and 5H. Upon completion flowback will commence on the 7H for approximately 2 days. I also performed field tests on the two sumps revealing a pH of 7 and chloride levels of less than 31ppm. Eclipse has not begun flushing the pad at this time. I completed walk around with Consultant Cristo Giannareas and witnessed the hot work being performed with the cellars and the trench being vacuumed of waters from the recent rainfall. Eclipse has not begun flushing the pad at this time. I received a phone call from Eclipse Construction Supervisor Doug Rennicker who stated they were installing a rain gauge today to monitor the expected rainfall and would be field testing on Monday to determine when they would begin flushing.

I also performed field tests on the two sumps revealing a pH of 7 and chloride levels of less than 31ppm. The third finger of the drain system has been installed with 2 more to be completed. Upon completion flushing of the north corner of the well pad will commence.
1415132820 2014-11-04 Casing String Witnessed No Violations On the day of my inspection I witnessed H&P Drilling Rig #531 drilled a 17.5 inch diameter hole to the depth of 1384 feet and Franks Casing crew ran 1363 feet of 13.375 inch 54.5#/ft. J-55 Surface Casing washing the last joint down. Halliburton was on site and gained circulation and pumped 10bbls of gel, 10bbls of fresh water and cemented casing with1385 sacks of class A cement 15.6ppg, 1.19cuft/sk cement, verified by mud scales. Displaced cement with 204bbls of fresh water had good cement returns of 44bbls, plug was landed at 540psi and pressured to 1050psi for 5 minutes and the floats held.

The well has screened off at the end of stage 52 of 56 with perforations bottom at 8669 feet and top perforation at 8587 feet. Mid-Atlantic was in route with a coil tubing unit to circulate sand out and complete stage with remaining 4 stages to follow. Walk around was completed with Consultant Daimon Girod.

Eclipse EHS Supervisor Bruce Carpenter and EHS Technician Ken Durst arrived where Mr. Carpenter stated that a vacuum truck would pull the contaminated fluids up and they would determine what activity happen with no supervision and then come up with a plan to remove contamination and remediate.

Field tests performed on the west fill slope drain revealed a pH of 7 and chloride level of 280 ppm. I also performed field tests on the east sump revealing a pH of 6 and chloride level of 333 ppm. I also tested puddles on the pad at global positioning system coordinates 40.039757 -81.235246 revealing a pH of 6 and chloride levels of 333 ppm. Gauges present read 1960 pounds per square inches on the casing and 2440 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater, a GP unit and a 400 barrel purge tank within containment and shared with the 9H, 5H, 3H and the 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales. All trenches have been covered, tracked, seeded and mulched.

Field tests performed on the standing water from the west fill slope drain revealed a pH of 7 and chloride level of 400 ppm.
1633406544 2016-08-18 Completion Testing No Violations On the day of my inspection I tested the flowing stream at Global Positioning System (GPS) coordinates 40.040271 -81.235808 revealing a pH of 7 and chloride of 1700 parts per million (ppm). All fluids from the stream are being captured in the sediment pond and pumped in the tank that was set yesterday. I then met with Eclipse EHS Supervisor Bruce Carpenter, Eclipse Construction Foreman Doug Rennicker, Eclipse Facilities Foreman Greg Offenberger and OEPA on Scene Coordinator Trevor Irwin. Mr. Carpenter stated that they are awaiting results from the sampling that Diversified Engineering had taken and that they are expedited. Upon receipt of sample results they will submit a plan to address the cut slope. Discussion with Mr. Irwin and Mr. Carpenter was held on a delineation plan if the samples come back positive near the marked wetlands.
1664651285 2016-08-17 Completion Testing No Violations On the day of my inspection accompanied by ODNR Customer Service Assistant Jimmy Wiles, I witnessed Diversified Engineering Services (DFS) was onsite and performed soil and water sampling to the pad and affected areas. I performed field tests on the sumps revealing a pH of 6 and chlorides of less than 31 parts per million (ppm). DFS dug into the north fill slope at Global Positioning System (GPS) coordinates 40.040271 -81.235808 revealing a flowing stream of water which further field tests revealed a pH of 6 and chloride of 1700 ppm. OEPA was notified by Eclipse EHS Supervisor Bruce Carpenter that there was a possibility of the wetlands to the west being affected. OEPA instructed Eclipse to perform tests to the wetlands. DFS completed their testing and Eclipse personnel installed a 500 barrel tank along the access road, removed the 6 feet by 100 feet area of contamination, dug and lined a sediment pond along with installing a pump to the tank. Eclipse Construction Foreman Doug Rennicker arrived and placed dye in the sumps and along the northwest corner of the well pad. Eclipse will continue to pump the sediment pond and provide ODNR a plan to address the pad after sampling results are completed. At the time of departure no dye had breached the north cut slope.
1748775107 2016-08-16 Completion Testing No Violations At 15:33 I received a call from Eclipse EHS Supervisor Bruce Carpenter stating that an area approximately 20 feet by 200 feet on the north fill slope of the pad has stressed vegetation. Mr. Carpenter stated that Eclipse is not aware of any spills that have occurred but initial readings with Quantabs revealed chloride levels were at 1700 ppm. Upon arrival at 16:25 accompanied by ODNR Customer Service Assistant Jimmy Wiles, I witnessed a 25 feet by 100 feet continuing to a 6 feet by 100 feet area of stressed vegetation on the north fill slope of the pad. Frac operations concluded on August 11, 2016 with ongoing drill out operations beginning on August 12, 2016. Field tests on Global Positioning System (GPS) coordinates 40.040222 -81.235692 and 40.040397 -81.236133 revealed a pH of 6 and chloride levels of 2825 parts per million (ppm). Additional tests in multiple areas on the well pad revealed a pH of 6 and chloride levels of less than 31 ppm. Eclipse Safety Consultant Mike Hartshorn place absorbent booms in the affected area. I placed call to Mr. Carpenter stating my findings that it appears to be brine related and questioned whether any testing was planned for the affected areas. Mr. Carpenter stated that they plan on starting in the morning to remove contaminated soils and installing a sediment pond at the base of the pad and remediating along with determining the source. He also stated that he would check whether any testing of the affected area was planned. I also stated that the remediation needs to begin as soon as possible to prevent contamination of a nearby wetland and tributary of Salt Fork Creek.

The 13.375-inch surface casing had 0 pounds per square inch, the 9.625-inch intermediate had 600 pounds per square inch, the 5.5-inch diameter production casing had 500 pounds per square inch, and the 2.875-inch diameter production tubing had 370 pounds per square inch. The production equipment consisted of five sand traps, five gas production units, a blow case, and one 400-barrel tank. The produced gas and fluid are separated and then transferred to the Borton CGF for sales. Identification was located at each well and on the tank.

I met with EHS Technician Tyler Bryant who provided me with the water sample results and stated that the secondary containment would be continuously monitored and that Eclipse would provide me with the soil sample results as soon as they were available.
