0034407964 2016-06-22 Completion Testing No Violations On the day of my inspection I met with Eclipse Completions Consultant Will Ochampaugh revealing that proper containment was in place for upcoming frac operations, Sweco production wellhead was in place and Nine Wireline was onsite preparing to run a gauge and junk run on the 5.5 inch production casing prior to running a CBL. Plans to complete runs on the 3H, 5H, 7H and 9H will follow with plans to open flow guns next Wednesday 6/29/16.

1418230843 2014-12-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P Drilling Rig #531 drilled a 17.5 inch diameter hole to the depth of 1393 feet and Franks Casing crew ran 1380 feet of 13.375 inch 54.5#/ft. J-55 Surface Casing washing the last joint down. Schlumberger was on site and gained circulation with 22bbls of fresh water, pumped 20bbls of gel, 10bbls of fresh water and cemented casing with1291 sacks of Class A cement 15.6ppg, 1.21cuft/sk. cement, verified by mud scales. Displaced cement with 200bbls of fresh water had good cement returns of 28bbls, plug was landed at 620psi and pressured to 1345psi for 5 minutes and the floats held.

Gauges present read 1165 pounds per square inches on the casing and 810 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater and a GP unit. Also present a 400 barrel purge tank within containment and a blow case skid both shared with the 9H, 7H, 5H and the 3H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales. Field testing of the sediment pond revealed a pH of 7 and chloride levels of 46 parts per million.

1969736983 2019-01-28 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with the production tubing opened and the production casing valves closed. The 13.375-inch surface casing had 0 pounds per square inch, the 9.625-inch intermediate had 1650 pounds per square inch, the 5.5-inch diameter production casing had 600 pounds per square inch, and the 2.875-inch diameter production tubing had 400 pounds per square inch. The production equipment consisted of five sand traps, five gas production units, a blow case, and one 400-barrel tank. The produced gas and fluid are separated and then transferred to the Borton CGF for sales. Identification was located at each well and on the tank.
2048160157 2016-09-07 Completion Testing No Violations On the day of my inspection I witnessed that Deep Well Services Rig # 4 (DWS) was currently running 2.375 production tubing which will be set at 8771 feet. I completed a walk around with Consultant Will Ochampaugh revealing proper secondary containment was in place and maintained and flowback operations have begun on the Wheeler 9H
