0129911028 2019-01-28 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with the production tubing opened and the production casing valves closed. The 13.375-inch surface casing had 0 pounds per square inch, the 9.625-inch intermediate had 950 pounds per square inch, the 5.5-inch diameter production casing had 600 pounds per square inch, and the 2.875-inch diameter production tubing had 370 pounds per square inch. The production equipment consisted of five sand traps, five gas production units, a blow case, and one 400-barrel tank. The produced gas and fluid are separated and then transferred to the Borton CGF for sales. Identification was located at each well and on the tank.
0546825856 2016-09-27 Production Wells No Violations On the day of my inspection accompanied by ODNR Southeast Regional Manager Brent Bear, I witnessed the well was equipped with Sweco wellheads and a plunger lift system. Gauges present read 1860 pounds per square inches on the casing and 3290 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater, a GP unit and a 400 barrel purge tank within containment and shared with the 9H, 7H, 5H and the 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales.
0568067736 2016-07-20 Completion Testing No Violations On the day of my inspection I witnessed Halliburton Services frac stage 31 of 45 with a breakdown pressure of 8187 pounds per square inch, an average treating pressure of 8612 pounds per square inch, an average rate of 87.4 barrels per minute and a total of 303970 pounds of propant pumped. I completed walk around with Consultant Dustin Moore revealing proper secondary containment was in place and due to mechanical problems with the fuel skid remained down and the job was fueled by diesel instead of natural gas.

1416774852 2014-11-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P Drilling Rig #531 drilled a 17.5 inch diameter hole to the depth of 1385 feet and Franks Casing crew ran 1344 feet of 13.375 inch 54.5#/ft. J-55 Surface Casing washing the last joint down. Halliburton was on site and gained circulation and pumped 10bbls of gel, 10bbls of fresh water and cemented casing with1240 sacks of Halcem cement 15.6ppg, 1.21cuft/sk cement, verified by mud scales. Displaced cement with 201bbls of fresh water had good cement returns of 40bbls, plug was landed at 550psi and pressured to 1050psi for 5 minutes and the floats held.

I completed walk around with Consultant Gilbert Aguilar revealing proper secondary containment was in place and due to mechanical problems with the fuel skid the job was fueled by diesel instead of natural gas.

I completed a walk around with Consultant Will Ochampaugh revealing proper secondary containment was in place and maintained.

Production equipment included a sand trap, a line heater and a GP unit. Also present a 400 barrel purge tank within containment and a blow case skid both shared with the 9H, 7H, 5H and the 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF for sales. Field testing of the sediment pond revealed a pH of 7 and chloride levels of 46 parts per million.
