0005581777 2014-01-13 Production Wells No Violations I am on site today to check on the clean up and to meet with the landowner who requested that I stop and talk with him. I checked the site and did see where soil had been removed and stock piled near the flare stack on the back left hand corner of the pad. I met with the landowner who wanted to ask questions on units and the next lease to his property.

I found that the pipe line is being installed today from the productionn area to the well. I was on site and returned calls to Anthony Miller about cementing and wait time. He also wanted to know about the cement bond log from another well.

0550437207 2013-08-22 Production Wells No Violations I am on site today to review the remediation of this spill site with Hal Miller. We checked the area of the spill and could find no stress to any of the vegetation at this time. The fluid which ran down over the hill could not have contained high chlorides. If there had been high chlorides over the vegetation after 5 days I would have found dead vegetation and I can not find any. The vegetation looks good. I advised Gulfport at that time no further clean up would be needed.

0677452448 2014-01-30 Production Wells No Violations I am on site today because Chris Anderson called me to inform me that a fire had occurred at the Clay Pad on the flare Stack and dike. It was a similar fire to the Lyons pad. They had a high fluid range valve fail and it allowed condensate to get into the flare line and cause a liquid fire. The fire was put out by the pumper on site and did not require a Fire Dept. The fire burned for around 15 minutes and did destroy the flare stack and liner to the small 10x10 dike which the stack is in the middle. The well is shut down until repairs can be made to the flare stack and the clean up of the area around the stack. The mist of burning gas and condensate remained on the pad and on top of the snow which will be easy to shovel up. No liquid made it out of containment so no further clean up will be needed.
1015951739 2013-08-28 Production Wells No Violations I received a call from Hal Miller and he advised me that the containment where our leak had been is being pulled up. I made it to the location as the trucks were moved forward and collected photos of the containment. The containment is being vacuumed up and will be removed so we can see if there is any evidence of a leak below. The top layer of containment had several holes in it which they said just happened. The next layer of containment was not seamed together and was just a cloth liner. The matting boars were next and with them out of the way I collected more photos. You can see the same black fluid on top of the lower liner as what was leaking out from under. I collected more photos in which you can see a few more holes which shows that fluid running under the liner is the same as the fluid on the liner. I also called Rick Triple to go over my meeting with Hull and explain where the spring was on the location.
1111466659 2012-09-07 Drill / Deepen / Reopen No Violations I have received a call from UDI # 121 to inform me they are switching back to drill on air. This well did encounter H2S Gas while drilling at 4800 ft and went to fluid at that time. The highest reading recorded was 12 PPM. They are having some drilling problems and have made a bit trip so they can try drilling on air. I advised that they continue monitoring for H2S. If levels approach 18 to 20 PPM they should go back to fluid for containment. They have multiple H2S monitors in place during this inspection. They are at 6300 ft unloading the hole in 300 ft stages. H2S readings are -0-
1190839426 2015-11-10 Production Wells No Violations Inspector Moreland and I are on site to review prior spills that occurred on this site and outside of the pad facility. With the inspection conducted today we found no recurrence of impact from prior spills. The pad had lush vegetation surrounding the production facility.
1225241069 2014-02-03 Production Wells No Violations The Flare Stack which caught fire and had to be replace has been. There is a new type of Flare Stack in place which has no flame visible to the eye because it is inside the stack. It also has a shut off which not shuts when the emergency shut off for the well shuts. That reduces a source of ignition.

1340278679 2018-10-01 Production Wells No Violations On the day of my inspection I am on location to follow-up ona phone call from Brad Burwell yesterday. While pulling out the original earthen tank containment Gulfport discovered some condensate and gotten underneath the containment through the years. Brad stated that the tanks do not have any signs of leakage. 17 roll off containers (40 cubic yards each) of material were removed from site. Brad said that Hall and associates were onsite to verify that all condensate had been removed. Largest excavation was a 15 feet by 15 feet by 8 feet deep. During my insepction I witnessed this area had been backfilled and graded. Operator stated they placed clean backfill material in one foot loose lifts and compacted it. A berm was not installed in this area after backfill and I mentioned to Brad that it needs installed to match the rest of the pad berm. Brad sent over emails and photographs. I forwarded them to engineering staff Paul Logan and Jerry Smith. This well has 0 surface annular pressure and no intermediate gauge. Brad was notified of the missing gauge. Prodcution equipment includes four vertical seperators, four sand traps, four GPUs, one combustor, one 50 barrel steel brine tank in its own earthen containment, as well as six 400 barrel steel tanks that have been removed from containment.
1347288495 2012-09-10 Drill / Deepen / Reopen No Violations UDI # 121 is setting 6907 ft of 9 5/8 casing. This is a two stage cement job with two lead and tail cements.Halliburton will be cemenitng. The H2S readings are - 0 - today. The casing and cement from todays job should remove any further H2S problems with well. Had about 45 barrels of good returns
1350563762 2012-10-18 Production Wells No Violations I received a call from Gulfport to advise me of a spill which occurred. I was advised that 25 to 30 barrels of { gelled fluid} mixed with frac sand and water has been spilled inside the 150 ft X 150 ft. contained area around the well. There was also an injury which sent one of the pumping crew to the Barnsville hospital. This same crew member returned to work within 2 hours. A vacuum truck was used to pick up the spill. How the spill occurred was the result of the failure of a high pressure 3 inch hammer union on a 3 inch steel line. This line was connected to the well manifold assembly. The hammer union failed on the threads portion of the union.
1376849013 2013-08-18 Production Wells Violations Noted I received a call from Hal Miller to inform me of a spill which occurred on the Clay Pad. I spoke with Hal and he told me that a 50 bbl brine tank had been over filled and that the containment dike around it failed allowing the brine to run over the hill 300 ft. He called the wrong number to contact me and by the time of my investigation there was no fluid to sample. The tank is now empty and there is no signs of stress on the vegetation from where the spill began to the area 300 ft down slope. The area where the fluid collected will need be excavated and removed to a landfill. Clean soil will need put in its place then graded, seeded and mulched until vegetation can be established.
1377618039 2013-08-27 Production Wells Violations Noted I received a call from Rick Triple. He advised me that there was fluid leaking out from under the containment on the west side of the location near the secondary containment area I had already set up. He stated that the chloride range was near 1000 PPM. I returned to the site and met Hal Miller. I conducted a quantab test and found the chloride range in the 340 to 350 PPM range. I did one last test and found one area 2 ft x 1 ft and it had a chloride range of 550 PPM. I advised Hal to empty the tanks above the leak area and to remove the containment to see where the leak began. They are to empty the tanks and move them to a new containment area. Friday the entire containment is to be removed and the area above the leak exposed. I will return to collect photos and samples if needed.

1512668916 2016-03-15 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of a plunger lift system. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

4 Line heaters 1 Combustor 1 Low-pressure Separator 1 50 bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A.

1530529478 2013-10-22 Production Wells No Violations I am on site today to check on the operation status of the well and to check the clean up status from the spill which occurred on8/27/2013 . That spill has been cleaned up and the area graded and seeded. Violation corrected.

2017286695 2012-07-11 Drill / Deepen / Reopen No Violations DNR Inspector Mark Moreland and I were on site to check the status of the site construction. Our inspection revealed that Rhino Construction has built the lease road and is currently working on the well site construction. We spoke with Tim Wilson (Gulfport Energy) and we advised him that Inspector Moreland was taking over the the well inspection duties in Harrison County.
