0194103776 2016-03-07 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells on site are producing off of natural reservoir pressure. Permitted wells 34067211040000 and 34067211050000 have not been drilled. RBDMS indicated that the wells on site are in producing order.
The production equipment on the pad consists of the following:

14 400 bbl. Production tanks 4 Producing wells 4 Sand separators

4 Line heaters 1 compressor 1 Vapor recovery tower 1 combustor 1 30bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A.

1086620611 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains four properly labeled producing wellheads. Production equipment includes five 400 barrel steel tanks in their own polymer lined steel containment, four GPUs, four sand seperators, four vertical seperators, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.
1349145170 2012-10-01 Drill / Deepen / Reopen No Violations UDI # 121 is running 18 5/8 inch casing to 606 ft. While going to bottom with the casing the rig had problems with fill up. They were able to get casing on bottom but while circulating bottoms up, the hole began to cave. About 15 ft of hole was lost. After trying to work casing and get back on bottom. It was determined that it would be better to cement it where it was rather than loose more hole. Nabors Well Service cemented the casing. See cement ticket for more cement info.
1358280030 2013-01-15 Production Wells Violations Noted I received a call from Gulfport Energy field agent to inform me of an oil spill at the Boy Scout production pad. I was informed that there was believed to be 14 to 20 barrels of oil on the ground near the production tanks. I next contacted the Uniontown Office and advised that I would be responding to the above spill. I contacted Inspector Grimm to assist me with the clean up of the spill and any problems which may occur. When I arrived I found that there was a spill but much smaller than advised. I found 2 to 3 barrels of oil next to the loading lines at the base of the dike surrounding the production tanks. There are 14 four hundred barrel tanks inside a dike which is 65 ft x 150 ft x 5 ft high. Peatsorb has been used to cover the loose or free oil in combination with oil booms to make a containment area. The size of the contained oil is a 20 ft x 20 ft x 2 inches thick area.Vacuum trucks are on site to vacuum up the contaminated combination of peatsorb and oil. I also advised that a roll off container be used to hold the rock and crushed limestone which will need to be removed to an approved landfill.
1358361675 2013-01-16 Production Wells No Violations I am returning to this site to check on the clean up of the oil spill which occurred Tueday 01/15/2013. The spill was contained to a 20 ft x 20 ft x 2 inch thick area in front of the dike where the load lines are located. The oil along with the peatsorb used to contain the oil has been vacuumed up an removed. The contaminated rock and crushed limestone has also been cleaned up and removed to an approved landfill. If you were to arrive today you would not know that a spill had occurred. Very nice clean up. No further inspection is required, this violation has been corrected.
1380552021 2013-09-30 Preliminary Restoration No Violations On the day of my inspection the well was in production and the site passed preliminary restoration according to 1509.072A.
