1377607603 2013-08-26 Production Wells No Violations I am on site today to check on any off site impact from brine spills which may have occurred on this pad from past drilling practices. I did not find any dead or kill area around the perimeter of the drilling pad or below.
1379591208 2013-09-18 Production Wells No Violations I am checking this site for low vegetation near the edge of the drilling pad. A check of the perimeter found good vegetation all the way around the pad. The production tanks are being spray painted during my inspection.
1421355826 2015-01-15 Production Wells No Violations I am on site today with Brad Burwell to go over the clean up of this site. The production tanks have been removed after finding a problem with the seal of the liner around the tanks. They had decided to work over the production tanks and reconfigure the liner. Gulfport wanted to test the area under the liner for contamination before replacing the tanks and installing the new liner. Jen Masters conducted tests by drilling bore holes down about one ft. deep and using a PID meter to screen for condensate. Her readings were high in several of the areas. Next Brad Burwell contacted me to advise that during a test for condensate, contamination was discovered. Brad said that they would be moving equipment in to remove the contaminated soil. Hull and Associates will be on site to conduct the sample screening using a PID meter. During my inspection today they have excavated the inside of the dike down 1 1/2 ft. x 30 ft. x 80 ft. The contaminated soil has been removed to a roll off container and the rest stored in a pile in the center of the dike.
1421773174 2015-01-20 Production Wells No Violations I arrived on site today to find that the excavation of the contaminated area inside the dike has been completed. Brad Burwell informed me that the condensate contamination inside the dike area has been completed. Hull collected confirmatory soil samples and has sent them to the lab. The production area will not be installed back inside the dike until the lab verifies that the samples are clean of condensate contamination. There is a pile of soil which has been excavated and is outside the dike area and covered by plastic. I contacted Brad Burwell and he said that the soil is contaminated and is on a liner. It is to be removed to roll off containers today.
1505242172 2017-09-12 Production Wells No Violations On the day of my inspection I came to follow up on a report of oil at the end of lease road from Gulfports Brad Burwell. It appears that a vehicle pulled off of the public road onto the lease road entrance and was leaking oil. It is my opinion that the oil spot has no connection to the well. Pad contains two proerly identified producing wells. Well is a plunger lift with with no pressure on the surface casing. Well is in production during my inspection. Production equipment includes four 400 barrel steel production tanks, two horizontal gas production units, one vapor recovery tower and a combustor.
1836051245 2014-01-13 Preliminary Restoration No Violations The site passed preliminary restoration according to 1509.072A. Well is identified but incorrect API number Gulfport was notified and correct number will be posted.
1851407420 2016-04-28 Preliminary Restoration No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells on site are utilizing a plunger lift system for means of production. RBDMS indicated that the wells on site are in producing order.
The production equipment on the pad consists of the following:

4 400 bbl. Production tanks 2 Producing wells 2 Sand separators

1 Vapor recovery tower 1 combustor The site passed preliminary restoration in accordance with ORC 1509.072 A.
