0707883621 2016-03-15 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of a plunger lift system. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

4 Line heaters 1 Combustor 1 Low-pressure Separator 1 50 bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A.

0936242469 2018-10-01 Production Wells No Violations On the day of my inspection I am on location to follow-up ona phone call from Brad Burwell yesterday. While pulling out the original earthen tank containment Gulfport discovered some condensate and gotten underneath the containment through the years. Brad stated that the tanks do not have any signs of leakage. 17 roll off containers (40 cubic yards each) of material were removed from site. Brad said that Hall and associates were onsite to verify that all condensate had been removed. Largest excavation was a 15 feet by 15 feet by 8 feet deep. During my insepction I witnessed this area had been backfilled and graded. Operator stated they placed clean backfill material in one foot loose lifts and compacted it. A berm was not installed in this area after backfill and I mentioned to Brad that it needs installed to match the rest of the pad berm. Brad sent over emails and photographs. I forwarded them to engineering staff Paul Logan and Jerry Smith. This well has 0 surface annular pressure and 100 on the intermediate. Prodcution equipment includes four vertical seperators, four sand traps, four GPUs, one combustor, one 50 barrel steel brine tank in its own earthen containment, as well as six 400 barrel steel tanks that have been removed from containment.
1179810166 2013-12-19 Production Wells No Violations I am checking on the clean up of the last spill 12/16/2013 which was 3 gallons of brine. I found that the peatsorb used to cover the free fluid has been scooped up and the area restored with clean rock. The material has been removed to a roll off container and is ready to transfer to Kimble Landfill.

1374244795 2013-07-11 Drill / Deepen / Reopen No Violations I had a call from Chad Shoffner of Gulfport Energy to inform me of a spill on the Clay drilling pad. Chad told me that the Gas Buster along with Flare Stack had caused a mist of soap and brine to kill vegetation below the Flare Stack. I informed ODNR office and Supervisor Rocky King . I arrived on site and found that only a mist had been applied to the vegetation and the soil. There was not enough fluid to collect a sample for the lab but I was able to get a chloride sample using a quantab. The range of chloride was 1400 P.P.M. I am meeting with AAA service to set up and construct a secondary containment system for run off and a future mist control.
1829430701 2013-12-16 Production Wells No Violations I am on site to check on a clean up which was reported to me on Sat. 12/14/2013 It was a 2 gallon spill which had glycol spilled from an over heated separator. It spilled onto the rock in front of the production tanks. The area has been cleaned up and the spill removed to a container which is on site. I did not issue a notice due to the fact that the area has already been cleaned up and the problem corrected.

1956313632 2013-06-12 Drill / Deepen / Reopen No Violations I returned today to investigate the misting release of brine and soap below the Flare Stack. I found that the secondary containmet system is being constructed and will be in place today. It will be to the far right middle of the pad looking from the entrance. AAA service has constructed and lined the containment system. The misted vegetation and soil is being removed to Kimble Landfill. The amount of material will be two rolloff boxes.
2070141352 2013-12-16 Production Wells No Violations I am responding to a spill which occurred a few hours after I left the location to check on the clean up of the 1st spill. This spill was a 3 gallon spill of brine which was in the same area as the last spill. A low pressure valve failed and allowed 3 gallons of production fluid to spill onto the rocks near the edge of the location. It has already been covered with peatsorb and there is no fluid present at this inspection. The area will need scooped up and placed in a container which already has some material in from the last clean up.
