Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five line heaters, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.

Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five GPUs, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Surface annular pressure was 200 psi and intermediate annular pressure was 0 psi.

1401898716 2014-06-04 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. H & P Drilling Rig #287 drilled a 17.5 inch diameter hole to 1953 ft and set 1940 ft of casing. Halliburton Energy cemented with 810 sacks of Class H cement at 15.8 lbs and a yield of 2.31. They had 20 bbls of cement returns.
1414789393 2014-10-31 Production Wells No Violations I received a call from Marcus Attaway of Hess Ohio and he told me that he was the Completion Supervisor for Hess Ohio. He was calling to inform me that there was a fire involving a Halliburton Pump truck. The pump truck was on site conducting stimulation of the well. During that operation it caught fire from a hydraulic line leak. The fire was brought under control with several fire extinguishers on site with the trucks. Cadiz Fire Department was called as a precaution in case the vehicle reignited. The Cadiz Fire Department remained on site and left after they were sure there was no further danger. There are 5 wells on this drilling pad, all are shut in. There were no injuries and nobody required medical attention. I contacted Supervisor Brent Bear to advise of the fire. The incident occurred around 12:15 PM this Friday.
