0584431406 2014-08-07 Drill / Deepen / Reopen No Violations On the date of inspection I was on site to check the status of the conductor hole drilling. Brett Farnsworth was the Company Man on site. At the time of my inspection Top Hole Drilling had spudded drilling a 26 inch diameter hole.
0694797188 2018-04-10 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 9 properly labeled wells that are producing. This well has a plunger lift system wellhead. Pressure gauges read as follows: Surface 0 psi, Intermediate 288 psi, Production 460 psi, and Tubing 231 psi. Casing hanger is plumbed into buried flow line. Production equipment includes twelve 400 barrel steel tanks within their own steel containment, three compressors, two line heaters nine sand separators, nine GPUs, a vapor recovery stack, and a combustor. One heater treater onsite. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
1084218545 2014-08-08 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Top Hole Drilling drilled a 26 inch diameter hole to 120 ft. and set 120 ft. of casing. Express Cementing Service cemented with 230 sacks of class A cement at 15.6 lb. and a yield of 1.20 and had good cement returns
1413370828 2014-10-15 Casing String Witnessed No Violations On the date of inspection I was informed by consultant Matthew Whitehead that the Nomac 72 rig had drilled a 12.25 inch diameter hole to a depth of 2420 feet. 9.625 inch casing at 40.00lbs/ft was set at a depth of 2408 feet. Schlumberger pumped 920sk of Class A cement weighing 15.6ppg, yield of 1.23. Cement was displaced with 178bbl of water, bumped plug, floats held. 55bbl of cement returned to surface and stayed at surface while I was on site.
1414769992 2014-08-08 Casing String Witnessed No Violations On the date of inspection I accompanied ODNR Inspector Mark Moreland to witness the cementing of the conductor string. Refer to Mark Morelands report for further details.
1415236354 2014-11-05 Casing String Witnessed No Violations On the day of my inspection I found Nomac Rig 72 had installed 450 feet of 13 3/8 inch casing. Shclumberger was doing the cementing. Circulation was established and 27 bbls of good cement returns were circulated to surface.
1767570713 2016-02-23 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067213530000, 34067213520000, 34067213510000, 3406721350, 34067213550000 and 34067213540000 are utilizing a plunger lift system. RBDMS production records indicate that the wells are in production. Permitted wells 34067213480000, 34067213490000 and 34067213470000 have not been drilled, but the tin horns have been set.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 6 Producing wells 6 Sand separators

2 Horizontal heater-treaters 1 combustor 3 compressor 1 Low pressure separator tower/Vapor recovery system unit. 2 NGL tanks

The site passed preliminary restoration in accordance with ORC 1509.072 A.
