I observed a plastic line connected to the pumping tee on the well was open ended and appears to have allowed well fluid to pump on the ground. I observed a skim of oil on top of a water puddle next to the well. The wellhead itself was covered in oily residue. The area within the dike for the tank at the well contained what appeared to be old oil contamination. I observed oil contamination at the end of the plastic line that drains the dike. I observed oil contamination at a flow line fitting where the line has been disconnected and could allow fluid to leak uncontrollably. The location for this pollution and contamination was mentioned in my report dated 1/4/2019. The pollution and contamination from the flow line leak is located at these coordinates: N 39.44228 W -82.40669. I took several photos during this inspection. I have made several attempts to contact the well owner Albert Poling but have been unsuccessful. I sampled water for chlorides near the well and tank battery with Quantab titrators. Within five feet of the well I sampled a pool of water with a low range Quantab which resulted in 8.5 Quantab units. That result equals greater than 645 ppm for chlorides. I also sampled water directly under a valve on the tank with a high range Quantab. That resulted in 7 Quantab units which equals 2,725 ppm for chlorides. I observed trash and oilfield debris laying around the well location including used cans, jugs, buckets, and cut up plastic line. I observed a bucket with crude oil in it sitting under the 1 inch pumping tee valve which was leaking into the bucket. At the unloading tank near the lease road entry I observed distressed vegetation and oily stained soil from what appeared to be a combination of a leak from the 3 inch load line and an inadequate dike around the 100 barrel tank. It looked as though the road ditch along Goat Run Honey Fork Rd. may have been impacted from this leak in the past. I left my business card with a note advising the well owner to contact me at the unloading tank. The well owner shall complete the following actions to bring well into compliance: 1) clean up pollution and contamination issues 2) clean up trash and debris. Time of inspection: 12:45 PM. |

I spoke with Albert Poling over the phone at 3:05 p.m. and explained the issues to him, and he stated that he will have the contamination cleaned up as soon as the ground dries out enough to move equipment in. |

It had caused two small scald areas 1.5 ft. by 25 ft. and 1.5 ft by 20 ft. As per my conversation with Mr. Albert Poling on 1/10/13 at 9:28 am, I told him that those areas must be remediated. I also told him that identification must be posted on the well, and that production records must be submitted for the well. I gave him Patty Nicklauss number at the Columbus office so that he could acquire the needed forms. |

Inspector Kip Thompson issued a compliance notice on 12/19/2017 for pollution and contamination. The inspection that took place on 12/19/2017 referenced oil contamination from the pumping unit motor as the reason for the compliance notice. A follow-up inspection on 5/29/2018 by Inspector Kip Thompson revealed a separate pollution and contamination issue at the well location regarding crude oil. My inspection revealed that neither contamination issues have been cleaned up. I observed oil contamination on the ground near the pumping unit motor and near the wellhead. I also observed brine contamination on the ground near the wellhead as far as 30 feet away going towards the 100 barrel tank. I tested a pool of water approximately 15 feet from the well with a low range Quantab titrator for chlorides. The test result was 3.6 Quantab units which equal 124 ppm chlorides. I also tested a pool of water with a low range Quantab titrator for chlorides at the tank beneath a 2 inch valve on the bottom of the tank. That test resulted a maximum 10 Quantab units which equals greater than 645 ppm chlorides. This well produces into a 100 barrel tank at the well location. I also observed a 100 barrel tank near the lease road entry point. Identification was posted at both tanks. I took several photos of the well location. Time of inspection: 2:00 PM.
