0154737019 2011-02-02 Annular Disposal No Violations FOLLOW UP TO NOV # 1296671350 Inspected by Andy Thomas. Identification tag faded on well head. Will need rewritten. Fitting on bottom of separator need repaired or replaced. Small leak starting to appear. Corrosion on bottom of knockout tank, and liquid underneath of it indicated that it may be leaking. Results of the high range quantab test revealed that the liquid under it was over 2000 ppm. Appears a fitting had been replaced or repaired on the bottom of production tank which allowed a small amount of oil to contaminate dike area. ID tag needs replaced, all producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank(s) as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Fitting on bottom of separator needs repaired/replaced. All surface equipment must be pressure rated to withstand operating pressures to which it is subjected. Knockoff tank needs repaired or replaced. All wells must be equipped so that no oil, gas or condensate is allowed to escape with the exception of gas flares. Clean up all oily material and dispose of in proper manner. At 3:20 PM spoke with Ray St. Clair via telephone and explained violations and what was needed to correct problem. He was already aware of the issues that the tank battery was having and had already bypassed knockout tank.

1266867243 2010-02-01 Annular Disposal Violations Noted An inspection revealed that gas is being vented from the well without the use of a flare. Inside the earthen dike at tank battery, pollution and contamination was observed at two locations. The first observed location was a fitting leak from a separator. The second location was underneath a knockout (standing) containing corrosion and found leaking at time of inspection. High Range Quantabs placed in fluid showed 10 units. A 1 line black plastic line was observed connected to bottom of knockout and no connection on opposite end of line. At conclusion of inspection, the well owner, Oil Enterprises, was notified in person of inspection results. The well owner states violations will be taken care of tomorrow.
