0286770210 2012-10-06 Drill / Deepen / Reopen No Violations Drilling; set and cement conductor casing.
0851378729 2014-06-23 Administrative Inspection No Violations Operator has reported that the landowner is interested in well as a water well. Well was never drilled into any oil producing zones and can be converted to a water well.
1349630944 2012-10-07 Administrative Inspection No Violations Contacted by operator to report that a fresh water mud zone was encountered at 265 feet. This caused loss of circulation and plugged the bit. Operator has tripped out of the hole. Then will trip back in to plug as lost hole with cement after using loss circulation material including straw to condition the hole. The rig will then be skidded to start a new hole. The operator will submit a revised application for skidded hole. The skidded hole will have conductor casing drilled and set through the 265 foot water zone.
1349879504 2012-10-10 Drill / Deepen / Reopen No Violations I arrived on site to inspect the drilling status of the surface hole. Killbarger Drilling Rig #5 had drilled a 17.5 in. hole on drilling mud to a depth of 198 ft. but was not currently drilling. I spoke to the tool pusher, Brian, and he advised me that the motor in the drawworks dropped a valve in the piston yesterday afternoon. The rig may be down for a couple of days until parts arrive and repairs are made. The driiling location was tidy and well organized. The lease road was well maintained and there was not any mud out on Graham Road. The pits were properly installed with liners and they were approximately half full at the time of my inspection. The doghouse was erected next to the rig floor and the drilling permit was properly posted. The original Stein #1 well is located approx. 20 ft. from the current hole being drilled. The original 17.5 in. hole was abandoned after drilling to a depth of 270 ft. and encountering substantial water. The hole was originally drilled on air and switched to mud after encountering the water. The water was circulated, but could not be overcome. The rig was then skidded to its current location and this well will be called the Stein #1A and have a different permit number. At the time of my inspection there was still a portion of casing protruding out of the ground under the pipe rack where the original well was drilled.
1626788159 2015-10-13 Final Restoration No Violations Agreement to convert well to a water well and transfer ownership to landowner on file in Columbus. Final restoration is completed
