0354444119 2012-10-24 Drill / Deepen / Reopen No Violations Inspect well and drill site. Killbarger Rig 5 drilling. Drilling for surface casing. Current depth of drilling is 440 feet. Attempt to collect water sample from the Sylvania Sandstone unit equivalent. No significant water was notes, thus a sample was not collected.
0889814063 2018-07-25 Production Wells No Violations Collect water sample TSB212 for analysis.
1105173086 2012-10-25 Drill / Deepen / Reopen No Violations I arrived on this drilling location at approx. 8:15 pm and found Kilbarger Drilling Co. Rig #5 had completed drilling an 11 in. hole on drilling mud to a depth of 1427 ft. and had placed 1371 ft. of 8.625 in. surface casing in the hole using a 10 ft. landing joint. A total of six centralizers were placed on the surface casing string on various joints as it was placed in the hole. Petroset arrived at approx. 8:30 pm and set up the cement head and plug. Petroset began pumping fresh water to load the hole and establish circulation to clean the hole of mud and debris. The pH of the water was tested and found to have a pH of 7. I observed circulation return to the surface and Petroset began mixing and pumping Class A cement with 3% CaCl. 50 lbs. of cottonseed hulls were added and pumped ahead of the cement. After pumping for some time, I observed some of the cottonseed hulls return to the surface. After continued pumping, I observed cement begin to return to the surface. Petroset pumped a total of 415 sacks of Class A cement and set the cement plug down at 9:15 pm. Petroset advised that they had a 1 bbl. return of cement at the surface.
1180450739 2015-06-03 Preliminary Restoration No Violations Identification is posted. A 210 BBL storage tank and one 100 BBL storage tank with separator within dike area. Area around well has been reclaimed, pits filled and seeded to grasses and clover. Baker pump jack with natural gas engine being used to produce well. No violations observed at the present time.
1351537393 2012-10-23 Drill / Deepen / Reopen No Violations I arrived on this drilling site location at approx. 11:00 am and found that Kilbarger Drilling Co. Rig #5 had completed drilling a 17.5 in. hole on drilling mud to a depth of 123 ft. They had also run 126 ft of 11.75 in. conductor casing in the hole. Petroset Cementing arrived at approx. 11:15 am and set up the cement head and plug. Petroset loaded the hole with fresh water for circulation and to remove dirt and debris. I tested the pH of the water with litmus testing strips and found it to have a pH of 7. I observed circulation return to the surface and Petroset began mixing and pumping 130 sacks of Class A cement with 3% CaCl. 50 lbs. of cottonseed hulls were added and pumped ahead of the cement. After pumping, I observed some of the cottonseed hulls return to the surface and a short time later I observed cement return to the surface. Petroset continued pumping until the cement was at the proper viscosity. Petroset then pumped down the cement plug at 11:55 am. Petroset advised that they had a 5 bbl. return of cement to the surface.
1351696705 2012-10-29 Drill / Deepen / Reopen No Violations I arrived on this drilling site location at approx. 6:15 pm and found Kilbarger Drilling Co. Rig #5 had drilled a 7.875 in. hole on drilling mud to a depth of 3323 ft. and was in the process of using power tongs to connect and place the 4.5 in. casing in the hole. Universal Well Services arrived on location at approx. 6:30 pm, but were delayed on getting back to the rig due to the heavy rain and mud conditions. Also, the power tong company was progressing slowly in running pipe due to the inclement weather and high winds. A total of 3306 ft. of 4.5 in. casing was placed in the hole. Six centalizers were placed on the 4.5 in. casing string at various joints as it was placed into the hole. At approx. 9:00 pm Universal Well Services was finally able to set up their piping and cement head and begin pumping fresh water to establish circulation and clean hole. After pumping for approx. twenty minutes, Universal began mixing the Class A cement and pumping it downhole. Universal pumped a total of 125 sks. of Class A cement and 53 bbls. of fesh water for displacement and landed the cement plug at approx. 9:35 pm. After landing the plug, Universal held pressure on the plug for one minute to verify that the plug held in place.
1501611899 2017-08-01 Production Wells No Violations Identification is posted and no observed violations at the present time.
1719611226 2012-10-29 Drill / Deepen / Reopen No Violations Inspect drill site prior to notification by operator that casing will be run and cemented. Drilling is complete, final total depth is 3323 feet. Bore will be logged and 4.5 inch production string will be run and cemented later in day.
