0290183589 2015-07-30 Orphan No Violations Today I met with Barry Bowers of Huffman-Bowers Co. and landowner Mr. Pearl Johnson to discuss the plugging of this well on his property. The conditions have not changed from the last visit and Barry and I discussed well information and how the well should be plugged. He is willing to provide an estimate for the plugging.
0291339199 2005-06-07 Production Wells No Violations Wellhead knocked off by contractor; mini Old Faithful bubbling gas out in a pool of brine. Darkish coloration near well bore may indicate some oil leakage also.
0600500607 2017-06-01 Orphan No Violations Orphan well field work with DOGRM staff members Tom Smith, Jake Glascock, Nate Lane, Josh Schiering, and Paul Carder to evaluate this orphan well site for surveying, hydrogeological, safety, environmental, and radiological considerations. Upon inspection, it is observed that the well is not actively leaking oil, gas, or brine at the moment, however the landowner states the well still regularly unloads brine and gas to the surface. There is still a large vegetation kill area around the well head area. The plugging project which includes this well is scheduled to be up for bid in September.
0716778862 2017-05-03 Orphan No Violations On location with DOGRM staff members Josh Schiering, Rob Lowe, Tom Smith, and Kris Roser to evaluate this orphan well location for environmental, surveying, and engineering considerations. The well is not currently leaking brine, however standing fluid is present around the wellhead and a large vegetation kill is still present. Photos were taken of the well location and access road.

1197210755 2011-12-05 Orphan No Violations Orphan well status inspection. Well casing is leaking brine and gas. Well pressures up then releases brine and gas in a fine mist. Kill area around well 15 feet by 30 feet, with a 8 foot by 40 foot kill area down grade from the well. Took photos. Brine on ground at wellhead maxed out high end Quantabs.
1269i10785 2002-05-23 Orphan No Violations An inspection of the well revealed that the well appears to be idle and orphan, there is brine and gas leaking from a hole in the side of the casing. No production lines hooked to well. According to landowner well has been idle for several years. The well sets in a hay field, kill area below well approx. 30ft x 150ft, contaminated soils will need removed and fresh soils hauled in. There is a bridge on the Twp. Rd. with a weight limit of 10 tons, and an old wooded bridge on old road, which will need to be replaced or repaired.

1300792794 2013-03-28 Orphan No Violations At the request of Michele Miller inspected this well. I encountered an electronic surveillance gate after I crossed the bridge over Pigeon Creek. Livestock was everywhere behind the gate so I retreated and went to Mr. Pearl Johnsons residence off Ray Road. He escorted me through the gate and guided me to the well locations. The surface casing was surrounded by water and saturated mud. On top of the casing was a swedge fitting going from approximately 9-inches to 2-inches. Not much left above this point. Measured chlorides with low concentration (30-600 ppm) Quantab. Measured 133 ppm chlorides. There is a circular scald area of vegetation around the wellhead and down slope from the wellhead for about 100 feet. I gave Mr. Johnson an exempt domestic owner packet. According to him he had not signed a right of entry form previously and would not sign one today even though I offered him one to fill out. There is no tank battery for this well. Access road is the Woodrow Hale Road (T-207) off route 327.

1424201024 2015-02-11 Orphan No Violations My inspection this day, with Jackson County Inspector Kip Thompson, was required to update the Orphan Well Program folder. We found this well in a pasture field, approximately 30 east of the woods. This well is equipped with 8 diameter casing (approximate diameter) and an 8 to 2 swage. There is a 45 x 15 area around the well that was filled with mud and brine water. It is visually apparent the this water had run downhill (north) in the past. There are buffalo pastured in the field which are drinking the brine water. They will need to be moved or penned up before the start of plugging activities. Access to the well is west off of TR 207, directly across from the barns, then across the pond dam, over a culver/bridge, through the pasture, to the well. There are no records in our database on this well. However, information taken off of the Ohio Fuel Gas map for this area indicates that it is a Hamden gas well that has not been plugged. I left word with Ian, the hired hand, to have Mr. Johnson call me with any questions. Pictures taken, SEM is 158.

1707691912 2007-10-11 Orphan No Violations Well is squirting brine under pressure out of a hole in the cement that is sealing the top of the 7 and the 2 tubing. Scald area is 10by 20. Scald area is probably much larger but the landowner has recently tilled and reworked the soil in this field. The brine maxed out a high end Quantab. The township bridge with a limit of 10 tons can be bypassed by a recently constructed stream crossing be side the bridge. No other restrictions were observed that would prevent a rig from getting to the site. The 2 tubing has a separate hanger. The 2 is full of brine but not overflowing at this time. Evidence suggests that brine has overflowed the 2 in the past. Well site is a flat field. A pond is located 200 from the well. A gravel road goes through the farm. A road will have to be built through the field to access the well site.
