0470148916 2017-09-14 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson rig 292 drilling the intermediate section of this well at approximately 4430 feet. During my inspection I did not find any visual tears or punctures in the containment. All of the chemicals were within containment. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. I did let the company man know that the containment around the chemicals was almost full to the point of overflowing. He indicated that he would have it drained. The pad drain valves are in the closed position and do not appear to be leaking.
0853973169 2017-11-20 Completion Testing No Violations On the date of my inspection I found a Halliburton Frac crew rigging down after successfully completing 54 frac stages on this well. They estimate that drill out operations should begin on the pad within 2 weeks.

Currently only the living quarters, frac tanks, and drill pipe have been staged on the pad. The pad is surrounded by healthy vegetation and the pad drains are in the closed position. The rig will be rigging up on permit 34-081-2-0711 first. The surface casing for this permit has already been set.
1498051581 2017-06-21 Drill / Deepen / Reopen No Violations I am on site today to verify that cement was brought to surface during the 13.375 inch cement job last night. I was not notified early enough to make it out for the job. I told the Ascent Company man that this is an issue and should not happen again. When I looked down the conductor annulus, I could see that the cement had not fallen back and was still at the same level as the valves into the cellar. The rig will be moving to another pad in a few days.
1797886932 2017-10-31 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on both wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 7 of 60 for permit 34-081-2-0711 and perforate stage 7 of 54 for this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1996548211 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 3 wells that arent yet labeled. Two wells, this one and permit 20711, have been drilled to total depth while the third well, 20709, appears to only have the conductor set. No pressures are shown any of the gauges on wellhead. Production equipment includes two line heaters, four sand seperators, two gas production units, and two 500 barrel steel tanks. Wells are not in production. There are also 2 empty frac tanks location. There is pipeline activity in the vicinity of the pad. Pad is vegetated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.
2056807585 2017-05-24 Casing String Witnessed No Violations On the day of my inspection I witnessed the conductor casing cement job. Precision Rathole augered a 36 inch diameter x 100 feet hole and ran 100 feet of 30 inch diameter casing. Casing weighs 118 pounds per foot. O-tex was onsite to cement with 330 sacks (70.5 barrels) of class A., 15.6 lbs per gallon. Additives included 1.5% CaCl and .25 lb per sack celloflake. Yield is 1.2. Displaced with 70 barrels of fresh water. 35 barrels of cement returned to surface. Mix water tested a pH of 7, cholorides at 38 ppm, and temp of 57 degress F.
