Enclosure gates were locked and secure. Tank lids were closed and secure. Secondary containment was present and intact. I observed the following safety/maintenance deficiencies: The wellhead enclosure gate lock was broken, and the load-line valves were not secured with locks as required. The ID signage identifies Great Plains Exploration as the current owner which conflicts with current record keeping indicating Osair, Inc. The owner must complete the following by May 27, 2016: Secure the load-line valves and wellhead enclosure gate with locks, update signage to reflect the current owner. The wellhead and battery are located via a dedicated lease road that runs south from White Road. The wellhead is comprised of a plunger lift system and battery (1 100-barrel lowboy steel production tank and 1 100-barrel lowboy steel brine tank) and mechanical separator in separate board-on-board enclosures. Fittings and connections at the wellhead and battery are secure. Load lines are secure and plugged. Process safety gear is present. 12 to 15 inches of snow cover obscures some surficial features and areas. The area surrounding the well and battery is mostly wooded and meadow with single-family residences beyond.

At the time of the previous inspection, I had observed the following deficiencies: The signage misidentified the well owner. The load-lines were unsecure. At the time of this inspection, the noted deficiencies have been remedied. I noted that the warning signage required by OAC 1501:9-9-05(E)(1) has become faded and unreadable. I contacted John Magnuson with Osair and advised him about the need for proper warning signage. He indicated that he would address it within the next week. I inspected the existing production elements and noted the following: the access road was well drained, the enclosure gates were locked and secure, the load-lines were plugged, locked and secure. The tank lids were closed and sealed. I did not observe any signs of leakage or problems. I walked the areas surrounding the wellhead and tank battery enclosures and did not observe any signs of impact or environmental degradation from the on-going production activity.

The well head location is in good repair and the site has been graded, seeded and vegetation is established. Board on board fence has been installed around the tank battery and the wellhead. The mandoor to the wellhead needs a lock and the well owner was notified of this issue. Landscaping has been planted but it has been heavily browsed and damaged by wildlife. The tank battery dike was in good repair and the lease road was stable.
