0006136823 2003-12-12 Plug / Plug Back No Violations Arrived on site this date. Bishop well services has a seven inch drill bit on upset tubing and started to drill out the well. At thirty three feet started bringing up wood chips. Tripped out and installed a wood cutting bit. Went back in and started bringing up more wood chunks. Broke through and continued to drill out the well.

0781292620 2003-12-17 Plug / Plug Back No Violations Arrived on site to find Bishop Well Services drilling at 535. Tripped out of the hole and found that the wood bit had been worn in a circle around the bit indicating a two inch hole in the obstruction. Went in with a tri cone bit in an attempt to drill around the obstruction. At 550 the bit began getting stuck. After three attempts to get beyond that point it was decided to plug the well where we were. When the tubing was tripped out it was learned that all three cones were broken off the bit. Tubing was run back into the hole and the well was circulated until the cementers arrived. The cement contractor was Formation Cementing. Cement was mixed with fifty pounds of cotton seed hulls and displaced to within 100. Pulled all but four joints of tubing and mixed cement to surface. The cement dropped and was topped off three times. A bell reducer and valve were installed and the well shut in. A total of 125 sacks of cement were used. The hole was eight inch to 150 and five inch from 150 to 550.

0913041112 2003-12-11 Plug / Plug Back No Violations Arrived on site this date to observe the drilling out of the abandoned leaking well by Gonzoil utilizing Bishop Well Services. The company had Daley Excavating dig out around the bent over pipe to locate good pipe that could be hooked on to. Found that the bent over pipe was just stuck into the well and not hooked on. Locate eleven inch pipe to drive down over the eight inch. Drove a fifteen foot piece of eleven inch pipe down over the eight inch pipe to solid material. Screwed an eight inch piece of pipe into the existing eight inch casing to bring it up to a working height. Tack welded the eight inch pipe to the eleven inch pipe for safety. Cut the eight inch pipe and welded on a coupler to enable installation of the BOP.

1668957078 2003-12-15 Plug / Plug Back No Violations Arrived on site and found Bishop Well Services still drilling out the well. Found a five inch casing seat at 150. Switched to a smaller bit and drilled down to 400 and hit solid which plugged the drill pipe and caused them to trip out to clean out the drill pipe.
1848420005 2003-12-18 Plug / Plug Back No Violations Arrived on site and found that the bell reducer and valve had been removed. Bruce of Bishop Well Services stated that there was approximately ten pounds of gas pressure on the well when he removed the reducer and valve. Tagged the top of the cement at thirty feet. The cement had set up. When water was dumped into the well to check for gas leakage it was learned that the casing was either holed or eaten away at the top of the cement and gas was coming in from the sand and gravel formation outside the casing. The Boardman Fire Department and Zoning Inspector were notified of the situation and it was determined that the well would be allowed to vent for the next week or two to see if the formation had been charged and the pressure diminishes or remains the same indicating the problem is solved or there is another driving force charging the formation. At that time the well will either be plugged the rest of the way and cut off or a permanent vent will be installed to relieve the formation pressure.
