1346969683 2012-09-06 Production Wells No Violations I inspected this well and found one small pump jack unit on the well. I could not determine the pressure on the casing nor the tubing due to the poor shape of the gauges that were currently installed on the wellhead. I found one 50 bbl. steel oil storage tank, one 50 bbl. plastic brine storage tank and one separator unit in the tank battery. An excessive amount of weeds were growing within the tank battery. Upon further investigation, I determined that the identification sign on the wellhead had the wrong lease name and well number on it. The sign currently identifies the lease as the WCAC #4 when it should be identified as the Tru-Fit #5. The owner of the well, Bob Workman of Tru-Fit Products, was contacted and verified that the well was mistakenly marked with the wrong lease name. He will have the maintenance man at the athletic club where the wells are located change the signage to reflect the proper lease name and well number . He will also have the weeds mowed. It was also discovered that the yearly production report for this well has not been submitted since the 1990s. Bob Workman was advised that he will need to file the annual reports for all the back years up to the current year with the ODNR. There were no other violations discovered at the well.
1349805406 2012-10-09 Production Wells No Violations I inspected this well and found one small pump jack unit with electric motor on the wellhead. I could not determine the pressure on the tubing or casing as both gauges were broken. No visible leakage was observed around the wellhead or piping and the area was well maintained. The identification sign on the well incorrectly identified it as the WCAC #4. The correct lease name and well number should be the Tru-Fit #5. The tank battery was enclosed with wooden privacy fencing and contained one 50 bbl. steel oil storage tank, one 50 bbl. plastic brine tank and one oil and gas seperator. The dike area was in good shape, but there was an excessive amount of weed growth within the battery. There was no identification sign on the fencing or tank. It was discovered that this tank battery handles production for this well and the Tru-Fit #6A, Tru-Fit #3 and Tru-Fit #1. I spoke with Jim Steger, the maintence manager of the Washington Court Athletic Club where this well is located and advised him of the proper lease name and number needed on the well identification sign. I also advised him of the need to have an identification sign on the tank battery. According to Jim, this signage will be made and installed within the next ten days. Jim advised me that this well produces gas that is used in the buildings of the athletic club and no gas is currently being sold. I also spoke to the well owner, Bob Workman of Tru-Fit Products, and advised him that no production reports have been submitted for this well since the 1990s. Mr. Workman said he will get the reports submitted as soon as possible so the records are up to date.

The diked area is located within a wooden privacy fence. A small fluid leak was noted at the piping connecting the two tanks that will need to be addressed. The tank battery is used by multiple wells; identification is posted for all wells utilizing it, however no permit numbers are listed. The well is equipped with a pump jack with an electric motor. Well identification is posted at the wellhead as required. Research into the production history of the well showed that no production has been filed since 1993, therefore the well must be plugged.
