0144691974 2009-02-24 Production Wells No Violations Well is in production and equipped with pump jack, rods and tubing.
1365534746 2013-04-09 Production Wells No Violations I inspected this well and found one electric powered pump jack on the wellhead. The gauge on the tubing showed a pressure of 55 psi. and there was no gauge on the casing so the pressure could not be determined. There was a small well identififcation sign posted on the panel of the electrical box next to the pump jack. No leaks were observed on the wellhead or on the adjacent piping and the area around the well was nicely maintained. The tank battery contained four 210 bbl. steel oil storage tanks, two 50 bbl. steel brine storage tanks, one vertical oil and gas separator unit, one gas sales chart recorder and one drip assembly. It was determined that only one of the oil storage tanks was in operation and the other three storage tanks were empty and disconnected from any piping. Also, the brine storage tanks were empty and not in operation and disconnected from any piping. The proper identification sign was posted on the operational storage tank and no leaks were seen on the tanks or piping within the tank battery. The dike was in good shape and the area within and around the tank battery was well maintained. It was determined that this tank battery also handles production for six other Leiby wells.
1377500943 2013-08-07 Production Wells Violations Noted This is a pumping unit well. The tank battery has standing oil inside as well as a large area of oil stained rock. Tanks are tilted to one side and there is oil under them. I contacted producer and he stated that they were switching out tanks and some oil escaped when they leaned the tanks. I advised him that all the contaminated rock and soil must be removed and replaced. To this point there has been no migration of any contaminants outside the dike area. I also advised them that they were to notify the inspector 24 hours prior to any work being done. Additionally, the tank signs are non-legible.
1382715692 2013-10-22 Production Wells Violations Noted Inspected production well on 10/22/2013. Wellhead is equipped with a pump jack with an electric motor. Well identification is posted at the wellhead, but it lacks emergency contact information. Tank battery consists of 4-210 bbl tanks, 2-50 bbl tanks, and a separator on a gravel pad within an earthen dike. Tank battery is used by multiple wells (as indicated by the well identification sign posted on it), but there is only one permit number listed on the identification sign. There is standing oil and contaminated soil and rock inside the tank battery, 1 of the 210-bbl tanks is tilted, and 3 of the 210-bbl tanks are leaking oil due to corrosion at the bottom of the tanks. Grass ~6-8 outside of diked area is oil-covered. Attempted to contact operator using emergency contact number listed on the tank battery and found that the number was no longer in service. Attempted to contact operator using contact number filed with the state; I left a message and have not yet heard back.

The tank battery consists of 2-100 bbl steel tanks and 1-vertical separator located within an earthen dike. The tank battery is used by multiple wells (5); well identification is posted for all wells utilizing it. The well is equipped with a pump jack with an electric motor. Well identification is posted at the wellhead as required. No problems were noted at the time of the inspection.

2037707806 2014-06-23 Production Wells No Violations Inspected production well on 06/23/2014. The tank battery consists of 2-100 bbl steel tank and 1-vertical separator located within an earthen dike. The tank battery is used by multiple wells; identification is clearly displayed for all wells utilizing it. All issues noted at the tank battery during the previous inspection have been addressed, and the well is back in compliance with the law. The well is equipped with a pump jack with an electric motor. A small amount of paraffin was noted on the ground immediately around the wellhead that will need to be cleaned up. Well identification is posted at the wellhead as required. Mr. Krauss with Hydrocarbon Resources was notified of this issue and will address it as soon as possible. No other problems were noted at the time of the inspection.
